Flexitrunk Nasal CPAP vs. RAM Cannula in NIMV

Could you please explain in greater detail why the vent senses a leak on the flexitrunk interface but not the RAM interface. Doesn’t it sense that there is missing flow in either case, at the expiratory end of the vent?

Great illustration of discrepancy of set and delivered and measured pressure. This ventilator can be calibrated with prongs and pressure at the interface (PEEP if) measured as well. Delivery of pressure with the RAM is very low.

So what happens if the baby doesnt have a pacifier in? At this point we would use a chin strap to prevent a leak from the mouth? I wasnt aware an open mouth can cause a leak

@@samroller5788 the mouth is a potential leak. In adults, bipap masks can be used to stop mouth leaks and maintain CPAP. That option doesn’t exist in the NICU so pacifiers or chin straps become and option. Unfortunately, cannula prongs are very far from the expiratory limb. If a baby needs to exhale CO2, they have to do so around the prongs or out of the mouth. In a true NCPAP interface, the nose of the baby is very close to the expiratory limb making it possible to exhale out of the nose without increasing the work of breathing. You can keep the mouth closed with a pacifier and baby can still safely exhale CO2
