Great lecture. The case answer is "A". Phrenic nerve injury acts as a restrictive flow pattern. Trouble with inspiration, can't get as much air in, decreased TLC. No scalloping in exp phase.

Thanks a lot. I will go .for 'A' for Case 15.. Diaphragm is a powerful muscle for inspiration..hence inspiration is affected (Restrictive) ... shifts its shape to the right of the normal.

The coving in obstruction loop is due to the first part being effort dependent which is normal so straight line while the second part is effort independent expiration which is affected in obst lung disease also called "dog leg/scooped out pattern"

at 12:05 i think the blue TLC line should reach from volume zero to volume 6 not from volume 1L to volume 6. otherwise youre showing fvc not tlc. otherwise great video!

Great lecture! I just wanted to point out that the back extrapolation volumes on those flow-volume loops are unacceptable and would be considered unusable per technical standards. I don’t mean to split hairs but I think it’s important that any viewers who may be involved in interpreting PFTs be able to recognize acceptable testing maneuvers. I look forward to watching more of your content.

Thank you for the video! I am a bit confused about maximal flow rate... our prof told us that the max flow rate happens at the equal pressure point which is a bit after that peak. But it's not the highest flow... how does that make sense? Thank you so much in advance.

i see in your flow-Volume graph that volume values in the horizontal axis goes from higher to lower numbers like 5 , 4 ,3 , 2, 1 ... But in my machine it is the opposite , it starts from zero and goes higher. is this problem ?how to interpret when i focus on this area or how to know if RV or TLC is increased according to my graph curve ?

in flow-Volume graph , what is the reference values of RV and TLD by liter?I see in your graph curve RV is 1,2 and TLC is 6 L . is this value applicable to everybody ?

My guess would be C sir , cuz the diaphragm contributes to respiratory but there are other inspiratory muscles that can compensate, so the graph will be similar to a normal graph in shape but reduced in all other aspects in term of amplitude. Am I right?

i have tried FVC test on myself, The inspiratory loop on flow volume loop did not arrived to starting zero point , it ended shorter on curve , is this problem ?while doing the FVC test , should we always close the curve of inspiratory loop completely bring to zero point by inhaling with stronger effort?

in flow volume loop , is the end point of expiratory loop is considered RV ?my machine does not calculate RV parameter, but my machine has flow volume loop graph. So from graph, i can myself know the real value by liter of RV , right?

If I have an increase of the FEV1/FVC ratio after bronchodilator which become normal (for example 75% before and 80% after BD) can we continue to talk about Obstructive Lung Disease ?

Bcoz phrenic nerve motor to diaphragm which is a prime msl of inspiration but quiet expiration is a passive process it is becoz of elastic recoil .

sir can u please explain why peak expiratory rate is 8 l/sec , you mentioned peak occurs in first sec and we know total lung capacity is around 6 litres how come volume be greater than 6 in peak expiratory flow rate ? its 8 l/sec and shouldnt it be less
