FMEA & FMECA: How to perform failure mode and effects analysis

Remember when Samsung had to recall the Galaxy Note 7 phone because it kept exploding? They ended up dedicating 700 engineers and built an entire facility just to solve this one problem. In all, the fiasco is estimated to have cost $10-$17 billion. Can you imagine how much better off Samsung would be if they had tested for a defect like this before launching the product? FMEA is just the analysis tool for this situation.
FMEA testing looks for all the errors that could happen within a machine, product, or process and mitigates the risk early on. It keeps teams focused on continuous improvement. Skipping this step is a great way to end up on the CPSC’s list of recalled products.

Building a PM to address a failure identified through FMEA is easy in Limble but what is missing is Limble’s ability to collect suspected root cause on the unplanned work order. If there is no data how can functional FMEA be conducted? Don’t get me wrong, Limble is by far the easiest CMMS for the end user to use but we need to add a mandatory failure mode collection system to be analyzed so we can build PMs.
