Nurse Injector here. Thanks for another helpful video. For my peace of mind, I don’t inject forehead and nose with filler. I recently started using ultrasound to map out blood vessels prior to injection and in dissolving filler. I think it will be the next best thing in aesthetics. Would you do a video on that?

These injections should be monitored somehow with an inserted camera so that they know exactly where they inject it . Basically I understand this is based in a process of estimation and speculation, therefore the mistakes are so much easier. But Dr. Pearce seems very diligent and knowledgeable, not all doctors are like this.

Dr another plastic surgeon online said that Botox in the forehead would lead to a drooping of the brows. Do you agree? He basically said you can’t touch it (except for resurfacing)

I have a spot of steroid atrophy on my forehead after having a nerve block years ago for headaches. I went to a cosmetic derm last week to get filler and she would only use saline to try to see if it would stimulate collagen there. Also, she did botox and now I am drooping on that side. Should I see a plastic surgeon instead to have this done? She said you would be able to see the filler if she injected in the forehead. I'm having regret that I didn't research this further before making the appt.

Often many practitioners are very nervous regarding filler to the forehead due to the risks, and it’s right that a practitioner only practices to their competency and training qualifications. I’d advise going back to your lady regarding your drooping. She maybe be able to support you more.

I still want to investigate filler in my forehead because of the unevenness from the atrophy. Do you think a plastic surgeon might be a better option for this type of procedure?

@@Cherylebreaux it truly depends on their experience of treating forehead with dermal filler. Not all plastic surgeons use dermal filler. But they all have an opinion on how best to treat the foreheads. I’d suggest speaking with both and see who makes you feel more at ease with the procedure. Kind regards Mary. I had Artecoll (permanent filler with PMMA-microspheres) injected into my forehead over 25 years ago and about five years ago experienced late onset granuloma. My dermatologist has been injecting it with steroids and Kenalog which show temporary disappearance but it always returns. Does anyone have any other suggestions besides surgical excision? Thanks in advance.

I have had success treating granulomas with a combination of oral doxycylcine and steroids. However, my patients were HA filler related (one was sculptra). I would consult a facial plastic specialist or aesthetic dermatologist with experience in prescribing these drugs and treating delayed onset granulomas. I have also heard of some providers using 5FU. You may need surgical excision. Best of luck. :)

Right above the brow (within 1.5 cm) as u said the supra trochlear and supra orbital are pretty deep. Would it make sense to inject filler rather superficially ( in dermis) if treating a Chicken Pox pit?

@Abc Def Thats right. Normally I would not do a filler at all for forehead. Just an occasional acne/ chiken pox pit, not responding to fractional CO2 / MnRF. There's a patient who has a pit there, so I was planning to fill there superficially intradermally. To be honest, the pit seems to be medial to the supra trochlear . As a matter of abundant precaution, I was planning to remain superficial since the nearby artery is actually pretty deep as it emerges out from supra trochlear notch.

This isn’t necessarily true. The risk for blindness is actually exceedingly rare, but the area is what makes it higher risk than others to inject. Only surgeons who routinely work around in these areas should consider injecting there.

@@nihilisticprophet6985 that doesn't seem possible from a pharmakinetic point of view, because to have an effect on the nerve it must be absorbed into it, at which point the toxin loses its ’key’to enter cells, and so is only able to work in that one neurone. 4 units is too little to affect all the neurones.

@@DrTimPearce I figured 4 unit shouldn’t be too risky but Ive seen many people reporting having such bad reactions. Some users suggested taking blood thinners would increase the chances of toxin going to other places in your body but there’s not much evidence to this claim. I suggest you look at some posts on Facebook in the two groups called “Botox Dysport (Side Effects) Support” and “Botox Almost Killed Me-Support Group”. The majority of the cases are people getting 30-40 units for their foreheads though, but 4 units are also reported. Posts like this drive me to look at toxin free alternatives like Thermirase which uses heat to deactivate neurons instead of toxin, but it’s so hard to find a practitioner, and it doesn’t treat crows feet.

Juvederm Voluma in the forehead is the best. Vollure works. Diluted Voluma as a base and use a cannula and fan out. Than after thats settles, patient come back and finish with needle to make it perfect. I was speaking of replacing Volume in the forehead. My forehead has like 15 syringes of Juvederm Voluma in it.
