If I am in company A and will join company B on 15-Sep-2021 (future date), what to write in the Company name? This form is supposed to be filled as the on-boarding process of company B (new company).

Form F is submitted to the company. 1. "In case of death of employee only current company is responsible to pay the gratuity amount to the nominees as declared in form F " and 2. " if employee is eligible for gratuity in one company (i.e. may be current/previous company or both) as per 5 years criteria and that gratuity amount is not yet paid by that company, then that company can pay the gratuity amount in declared proportion to the declared nominees in form F. (i.e. If employee has completed 5 years in previous company and that gratuity amount is not paid to the employee, here if anything happens to the employee then nominees as declared in form F with that company will receive that gratuity amount)". As you are currently working in Company A , you can submit form F to Company A. Because you are employed with Company A till 14th Sep 2021 and they will keep in record for reference in case they need to pay your unpaid gratuity amount to the nominees. And yes you are correct, form F is supposed to be filled as on-boarding process of your new company B . Here , you can again submit the same form F to company B on 15th Sep 2021 so that company B will keep it in record for reference to pay the gratuity amount to the nominees. Please note that "nomination form F is employer specific" and not centralized.

@@billambilla If you are writing on word document .You can convert the word document to pdf and can then take print out or you can take the blank pdf print out and then fill it using pen. If they are asking specifically your Aadhaar card you can submit.
