Could be that he had the knowledge but couldn’t back it up with the practicality, seems like he was possibly too nervous or stressed to be a good doctor when push comes to shove. Also could be that he just lost whatever had gotten him through med school when he was trying to learn as an intern. Stress can cause a lot of difficulties for people and come out in different ways.
