There's actually a pretty interesting reason behind that, which you might to know, so I'm here to inform you, and if you do know, too bad. I'm telling you anyways. In order to keep the facial scar prosthetics on, Heath Ledger had to keep prodding them with his tongue. Because he knew it would look weird if he was just occasionally using his tongue to keep the scars from falling off, he decided to just make it a facial tic of the Joker's.
