WHAT HAPPENED
This 59-year old jumper fell in love with skydiving after her first tandem, so she came out to get licensed! (We love hearing stories about people who get into the sport later in life!) On her first Instructor-Assisted Deployment (IAD) jump, she was a bit nervous. After taking her time getting out of the plane and setting up - nothing wrong with that! - she exited, but the instructor appears to throw the pilot chute up, and it end up clipping the tail! Thankfully, it released as her main deployed. Aside from some line twists and some missing paint on the tail of the plane, everyone was okay.
WHY DID IT HAPPEN
Instructor Error
While the student's exit wasn't the best and their body position was as spread eagle as one can get - to the point of resembling a starfish - that wasn't the cause of the issue here. A slow motion viewing of the video shows the instructor releasing the pilot chute and all of their fingers are splayed out pointing at the tail. By releasing the pilot chute towards the back of the plane, they were the ones whose actions led to this incident.
HOW COULD IT BE PREVENTED
Follow the Manual
The US Parachute Association Instructional Rating Manual (the "IRM") notes that IAD Instructors "must deploy the pilot chute out and down in a manner that ensures clearance from all parts of the aircraft." That didn't happen here. Effectively, this instructor looks like they almost threw the pilot chute at the tail rather than down and out. Thankfully, however, the jumper, the plane, and everyone on board was okay. We're also glad to say that, despite this rocky start, the jumper followed through with getting her license!

Jump pilot here! I would like to say that there are a lot of comments about the coach/instructor being at fault. Why yes he should have thrown it down. As a pilot you should always use 1 notch of flaps (he did not have any) when doing IADs (or instructor assist deployments) reason…. When u add a notch of flaps it creates a down wash that pushes that chute down past your stab

I've JMed students a few hundred times using IAD in Canada with no incidents. When it's all you have ever done as an instructor it is not so mysterious or scary. You do want to throw the pilot chute down, not just release it from a high position like in this video.

When doing IAD deployments, it's important for instructors to remember that the horizontal stabilizer is there to assist them in the event that the instructor does not know how to throw the PC at a downward angle. Everyone knows that.

😂😂 but fr the pilot should have added a notch of flaps so that the IAD didn’t even stand a chance. I’ve had out of my jump plane terrible instructors drop for students. I have a clip of a instructor who let go b4 their student even let go of the strut (jump pilot here)

I never even heard of IAD until _after_ AFF and _after_ I got my A License 😆 Feel like I probably made the right choice.

Love the Protech helmet. i had one for at least 4yrs. 25yrs ago. I watched a very experienced jumper exit a Queen Air on a hop and pop and his bridal took off most of the paint off the leading edge of the port tail. I was spotting and said four letter words. Don't. do that.

A few issues here. First the instructor should have called for 10 degrees of flaps. Second, should have thrown the pilot chute down and back. Third hanging exit puts student a little lower. Spanky, IAD I/E. I've done a few thousand of these and AFFs.

I dont know what all the fuss is about. Worse case scenario is the canopy gets caught around the tail breaking it off and causing plane to go into a spin and or dive with instructor and pilot struggling to get out.

the instructor has more chance of getting away from the plane as they are already outside but those left inside would really struggle to get out before impact assuming the pilot even has a parachute. The exit is at the front but loosing the stabiliser would likely put the plane on its side and into a dive making it difficult for the occupants to get out of what would now be a fairly alien environment with the aircraft on its side and effectively in zero G trying to climb down to the exit to get out. They're below 4000ft, that's gonna give them less than 20 seconds to get out and deploy

What could happen if the bridle didn't manage to get clear? I'm not really sure where the first point of failure might be. Something in the chute ripping apart? Breaking off a piece of the plane's tail? The jumper getting stuck and dragged behind the plane?

I saw that but with a static line, that ripped the whole empannage of a Cessna 206, and killed I think four.

IAD - I’ve not seen this IAD before and I’m not so sure I like it - it’s like a Gerry rigged static line / problem waiting to happen

I would bet it came out of that type of mind set. Since the student was wearing a pro tech helmet maybe just an old fashioned DZ

Pilot has to tilt the nose down, tail over the horizon. Slow descent. Shorter bridle. Throw under the plane. Not an instructor, just the info I heard from way more experienced people.

With the nose down, you are going to have a very high airspeed. That sounds like it could create problems of its own.

Im sure IAD can be done safely, but this doesn't look like the way. I see so many likely potentials for things to go bad very quick.

If you want to help the skydiver in this way, hold the pilot chute in your hands until you see the parachute chamber coming out of the backpack under you and the plane and then just release the pilot chute.

Huh. That's interesting and makes total sense. I wonder why in over 75 years the USPA has never figured that one out. I think you should hold onto the pilot chute the entire time though, just to be extra safe when the parachute chamber comes out of the backpack.

i made quite a few(10?) static lines jumps before going AFF. I was going to quit. i was that frustrated.

I wanted to be a solo jumper, but after watching the grim video on how to jump and what all could go wrong....I decided against it. I'll use my third jump this July with my dad and wife as tandems. Skydiving was amazing that first time, but the second was a little lackluster. This third one might be my final unless I decide to skydive in Hawaii. That seems like a beautiful place to skydive.

Worth noting also it gets way better when your licensed and can actually really enjoy jumping with others and trying new things. I didn’t really enjoy my tandam or the first 3 or 4 levels to be honest. But once your over the hill of fear and having to concentrate so much because you don’t want to fail levels it gets so much better trust me.

@@sergeig685 nahhhh, bad comparison. Way more risks involved with skydiving. I just don't trust myself or at least don't want to encounter the situation of an issue with so much adrenaline involved. I know myself and know I'd give it up after my first incident anyhow. I went in fully excited to go solo, but then just had to accept being planted on the ground is an easier way to live. Heh. My close friend had a double-out incident very early on in a solo jump, which probably is not very common, but that's just too close to home and always in the back of my mind when thinking of skydiving.

What year was this jump? I can't imagine why anyone jumps IAD instead of AFF, I don't even think this was an option at the two DZ's went too. I've only ever did AFF, so I have no idea how static line or IAD or tandem feels, but I like to be in control of my own life, so this would never fly with me. This could have ended in life ending tragedy of everyone on board w/a crash at worst or her death. Life is so fragile people. NEVER get complacent in skydiving. Take the next load if something is wrong or you are not feeling it. The sky will always be there. Safety first. Safety always. #BlueSkies

AFF works well for turbine DZs where you can get to 13k in 15 minutes. Small Cessnas take ages to get to full altitude and you can only fit 1 AFF student with instructors in the plane but they can put out 4 IAD students on a single load and get to the right altitude in a fraction of the time it would take to get to AFF height. Given there's much less to teach in terms of freefall skills for the first jump, IAD / SL focuses much more on canopy in my experience which is generally the bit that will hurt you so I believe it is actually the safer method of teaching, just not practical for turbine DZs.

at our DZ we have a black tape at the end of a crossbar, and you MUST go beyond it. I am a beginner but even I can see the problem here. Not to mention that a student could hit the side step. Shame on the instructors
