@@Akio_da_fox5530 the hearts has a number that is constantly getting lower. The number represents your health being 1 the lower health digit meaning that missing a note is instant death. The health will only go up if you are hitting notes constantly without missing but it will start draining as soon as you stop hitting notes leaving you at 1 again

It's a game series made by Running With Scissors. The mod is referencing the second game which is the most popular well beloved by the community. Basically the game is a first person semi open world where you do mundane task like, pick up your check, asking people to sign your petition, buy suspicious milk from al Qaeda while waiting in a very long line. It's up to you whether or not you want to go apeshit and murder everyone, basically, going postal.

Postal is a video game series mostly known for being extremely offensive and violent. The mod in the video is about Postal 2, arguably the most popular one in the series. Postal 2 is about you are a guy who lives in Paradise, Arizona. The goal is to complete chores that you were sent to do by your nagging wife, or just regular things like cashing in a check. You play through the week, Monday to Friday, each “level” being a day. The catch is all kinds of chaos ensues just by doing basic tasks such as buying milk. You can act however you want in Postal 2, you can be violent or try not to kill anyone. You should probably read reviews on Steam and see what warnings the game has if you ever plan on buying Postal 2 as it’s not for everyone.
