This is part five in my series on custom foot orthotics. In this video I explain my favorite foot orthotic. This insert is based on engineering principles. It is made to match the arch of the foot and is calibrated to weight and flexibility. The foot is a masterpiece of engineering and I have found that an orthotic that is based on the principles of engineering works the best at solving many conservative foot issues.

These videos are an incredible service. I’m a lifelong endurance athlete recently dealing with PTTD. I’m also an emergency room doc. Understanding gait mechanics, how to rehabilitate to A functional state, and what orthotic strategy to utilise has been really complicated. I actually started making my own silicone molded arch supports and forefoot and hindfoot corrections while awaiting my custom orthotics. I have a new understanding of how broad podiatry and allied health can be and really appreciate all you and your colleagues do. Cheers.

OH, I had a "press mold" done and it was TERRIBLE! It didn't work because my big toe has limited motion and it was a mess. The podiatry office didn't have a clue and I complained to the State Board and I wasn't the only one ...This method isn't for everyone, in my opinion, and I don't think some doctors know how to even do this correctly! What's the material used for the actual orthotic?? Frankly, I found Natural Foot OTC orthotics and they helped me fit them with a free exchange and they FANTASTIC!

If you support the ball of the foot on the plaster it works better. Never had good results with a crush box

Material science and biomechanics are very important here. This makes alot more sense now. Reachout to the PNW Boot Makers Nicks, Whites, JK, Wesco, Drews, Franks and look into how to incorporate the lasts with the material design of leather to make custom ortho boots.... Would be some great videos..

Bingo! My Nicks' Tactical with the HNW last are amazing, given they're mostly rubber with a simple veg tanned insole. My JK 300X's are a bit harder on the heel but are much lighter.

Hi doctor, this is insightful indeed! Do you also recommend exercises that patients can do to naturally increase foot arch besides using an arch support? Hope you can make a video on it!

all insoles that i have had made, the had one major problem.that i keep running into. that is, the arch is too far back. i went through upstep, and two different podiatrist in my area to have them made(pedalign). it feels like the arch of the insoles pushes into my heel. oh and for get about using a ''post'', oh my god, my whole equillibrium was off, from my knees, hips,back, and neck was in pain for days after wearing them. the last time i had a pair that i "broken in", it took 6 months of literally wearing them every single day. non of this wear for 1,2,3, hour bs. pain doesn't just go a way after taking them out it takes just as long for the pain to subside. i got a riddle for you: what feels like an ingrown toenail but isn't? big toe sharp pain at the tip of toe but towards the right of nail . right where it would be if i had ingrown nails. when i push down at the spot it feels sharp. ingrown toe nails were remved back in april for last year -_- still have the sharp pain

I recently had custom orthotics made using the sock style casting. So far they are better than over the counter but I am not absolutely pleased with them. I still don’t think the arch support is quite high enough.

Sock style casting does not incorporate the floor. There is no way to measure the appropriate arch height. Slipper casting relies on posting all of which I have discussed in prior videos.

@@frugalfoot4973 I did see those videos of yours. Unfortunately the sock/slipper is the process my former podiatrist used. I’m hopeful that these orthotics work and that I don’t have to try this all over again. Much less pay for this all over again

In the video the person is wearing socks while doing the impression? Is that what you'd recommend?

Operator's preference. There is no right nor wrong. It is just how I do it.

@@frugalfoot4973 Excellent. Yup, I just did my impression today and sent it out. I used this method and it makes logical sense vs standing into the foam (collapsing arch in the process). It's odd how the orthotics companies don't suggest this. It's beyond preference and just logically seems right vs wrong method. Either way...thanks for the vid! Glad I stumbled across it beforehand. :)

Have you ever discuss double depth shoes they allow you to put extra thick insoles --one particular type is made by Drew but I’m looking for an extra depth boot I can wear on concrete

Your link tot he lab no longer is live. Second, what is your opinion of imaging the foot with digital scanner?

Thank you for the notification on the link. As for my opinion regarding imaging the foot with a digital scanner: This is very similar to walking through a sprinkler on a sidewalk. It may tell you where pressure points are but it does nothing to indicate the height of the arch. I don't use them.

Well, that explains why Upstep didn't work for me; the instructions on KZbin have you stand in the impression kit, doing exactly the opposite of how you captured the impression. Also Upstep uses what appears to be PLA, since these have the telltale "fingerprint" or "tree rings" lines and have very little to no flex. Even with a resign they didn't fit in my boots, so there was ~$200 down the drain. Currently waiting for a refund. This is why you need a professional, not some chump company advertising on Facebook.
