There's no error in Code. Also Log out and clear your browser Internet cache. Close the browser and reopen it. Make sure cookies are allowed/enabled in your browser settings so the CAPTCHA will appear.

Mismatched token error occurs when you input diffrnt userid and and tokenId. Just put same credentials in both of ghe fields like Userid: [e-mail] TokenId: [e-mail]

@@Nowdemy i want to thank you first of all for this great work you have done. To solve the captcha issue is just a matter of deleting the ";" to enable the gd extension on the PHP.ini file "extension=gd"
