For my original, when I was at my appointment they took a half dozen or so photos at different angles of my head/face. They were nice and the process was quick. I did my app online and then it prompted me to make an appointment. I made one and did the rest there (fingerprinting and pictures, etc). I think my original only took about 10 days to get to me.

I just got my TWIC card earlier this year. Truckers need them to enter most ports.
