Pee-wee Herman wasn't actually a huge name yet in 84. Paul Reubens had done a live show as Pee-wee, and it had been made into an HBO special, but the Pee-wee movies and TV show hadn't happened yet. At the time, he was arguably more widely known for his bit parts in Blues Brothers and some of the Cheech and Chong movies.

The drilling a hole in a head thing is probably a reference to trepanning which was used as a type of early brain surgery and also to promote psychic/occult powers, which is probably why it's in this script.
