It's in an old pitch, the most recent pitch change was in the 50s. There is talk of raising the pitch to A=440

Well, for an object that was made over 200 years ago (while Napoleon Bonaparte was still alive), I think the condition is actually very good. But to answer your question: Yes, a musical box can go out of tune. The reason is often the decomposition of the lead weights, improperly replaced dampers or excessive cleaning of the comb. Retuning is extremely time-consuming and has to be done by filing the reeths.
