Actually if I'm correct, Quake (released in 1996) was the first fps game to use a grenade launcher, and Star Wars Dark Forces (released in 1995) was the first fps game to use hand grenades (which look different, and are actually called thermal detonators, but still, it behaves the same way as a grenade). That's all I really know.

Well... one could possibly argue that Wolfenstein 3D (1992) had some stealth elements to it. You could sneak behind guards and kill them that way, and as long as you use a knife then nobody else would be alerted.
