Scurvy isn't a disease, it's a condition. The difference being the method of contracting. You get scurvy from not having enough vitamin C in your diet. A person with scurvy could sneeze in your mouth and you'd never be at risk of catching scurvy. You could develop scurvy on your own, but that would be completely unrelated. That all being said, I don't think Oda is making that distinction. In modern One Piece I doubt there will be any real world conditions either, same as the diseases.

I think it's gonna be limited to infectious diseases and have a time limit because of that. A full immune respone take about ten days, in One Piece it wil be faster depending on how strong you are. But still faster than irl. That would be my guess.

Even if it wasn't stated, i believe the reason Doc Q chose such a silly disease to attack the heart pirates is because he can't control the spread of it, and Blackbeard seemed to bank on his knowlege of Law's powers, which would include teleportation. So thousands of things can cause cancer. Merely healing from an injury can lead to cancerous growth. So in order for this fruit to ENSURE cancer, is he would have to cause continuous nonstop damage/mutation to a person's cells until one has lasting traits. Most cancer cells die on their own or are removed. It's about developing traits that allow it to continue consuming nutrition and evading the immune system that matters. So Doc Q would have to engineer a virus that can alter a cell in such a way.

Heres a thought, doc q holds the key to choppers dream. If haki creates antibodies for a disease inflicted by the fruit, then the fruit is "the cure for all sickness". So think of it like this lets say a strawhat was to contract the same sickness that killed choppers father thanks to doc q's fruit then someone releases their haki and cures the disease, their blood would have the antibodies capable of fighting the sickness thus creating a cure. The cure for every thing can be as simple as a blood transfusion.

My guess on the actually parameters of the sick sick fruit, is that it works like a mirror of the horm horm fruit, effectively gaining potentially infinite range via transmission, in exchange for a lack of control over said transmission and any cure spreading just as easily. So Ivanokov needs to hit you up close and personal, but then you are stuck with whatever he did to you(unless law's haki based counter would work for that but then you need enough strength and skill with haki for that) but Doc Q just needs to expose you to the disease and it will propogate. I bet whatever the Blackbeards did to breach the Heart Pirate's sub carried the disease, just inflicting disease by thinking about it would be too powerful. and not good for story reasons.

What if, when Law broke through the feminization disease with his haki, he produced antibodies that now make him immune. So Doc Q's weakness is that, the more he fights against a strong-willed opponent, the less diseases would be effective against them. With that logic, it wouldn't be wise for Doc Q to immediatly hit the opponent with his most dangerous disease. He could give Luffy cancer, and then Luffy could use haki to become immune. Maybe that's Doc Q motivation as a dark foil to Chopper, for example. Maybe his philosophy is curing by infecting people with horrible diseases, and if they survive it means that they are strong-willed enough to deserve to live.

Awakening the fruit might enable the user to inflict long lasting diseases. Maybe thats what happened to rodger. The diseased can save himself from symptoms by using haki but keeping haki up all the time not to get killed by the disease is draining your live to a point that you will loose lots of life expectancy.

Even when restricted to One Piece diseases its pretty scary considering white amber lead and sapphire scales would be possible. Sapphire Scales in particular would be similar to having your shadow removed by the shadow fruit.

What if to create a certain effect he needs a sample, so to transform Trafalgar into a women he run some samples on a female in Black Beard’s crew. So if he wanted to create a disease that gave the effect of another devil fruit he would need to run samples on the owner of said devil fruit. It would allow that power to have infinite possibilities but also limit how op it is.
