Basically the difference between grandma and aunt is a single syllable. おばあさん/obaa-san vs おばさん/oba-san There's even slightly different kanji to use for if your aunt is your parent's older (伯母) or younger (叔母) sibling, but the pronunciation is the same, it's mainly for writing.

Oba-san vs Obaa-san is one of the trolliest parts of basic Japanese that can result in the most accidentally insulting outcome. To native speakers the distinction might seem obvious, but English doesn't have the same difference of emphasis to differentiate words, so people will most likely just say the wrong thing and have no idea. I just always slow down and over pronounce the baaaaa part for grandma or old lady, and for aunt? I just say it super quick and pray to god they don't look offended.
