It starts with a needle sized hole that goes all the way through. The organs shift and grow around it over time. If they were to impale him with that hole from the start yeah, he'd die, but the human body is pretty awesome. Obviously it can't contort to this extent in real life, but people have had things lodged inside them and the body learned to "grow around" it. So it passes anime logic at least lol. Edit: actually you're right, makes no sense because if that hole goes out the back, where is his spine lmao

@@MrGksarathy Well the problem I'm seeing is that if you have a stick that goes from your stomach through your back, the internal organs can grow around it as it gets bigger, but the spine can't. So as the stick gets bigger, you either have to make it move the spinal cord out of the way, which would literally make it bend and eventually break, or you need two sticks, one to the left of the spine, and one to the right. If that happens, a hole from front to back could be made, but then your exposed spinal cord is just showing, so that wouldn't work either.
