What are the frequency ranges affected by the bass, mid, and treble sliders?

That depends upon the type of aid...they have five levels of tech that use with with multiple frequency variations and many bands. It's an unanswerable question.

I have the resound 3d app on my phone but do not see the slider after selecting sound enhancer.

Without knowing specifics, it could be when you got the hearing aid. This same app has been used for approximately Five SETS of hearing aids from the ReSound Lynx 3D, Omnia, ONE, Quattro and Nexia. Now it could also be that you have a lower level newer hearing aid. Don't know which.
