Prep medic, if you are not trained to do a chest decompression and don’t feel comfortable carrying chest seals, what could you do to a sucking chest wound to stop the bleeding but not allow tension pneumothorax
@dtreece224 жыл бұрын
God gave you hands for a reason
@SBRanger34 жыл бұрын
Why would you not be comfortable carrying chest seals? They are just sticky squares with one corner for venting....there is nothing invasive about them. Where I work that is a BLS skill that EMT-Bs can perform. If the patient has a sucking chest wound, you need to stop it from progressing as quickly as possible. While decompressing the chest will be an intervention carried out by advanced providers, stopping that pneumothorax from progressing to a tension pneumothorax, if possible, is critical and will most likely going to be life-saving (granted the patient does not have any other critical injuries that need interventions). Chest seals will definitely help with that. Gauze and other similar items will allow air to pass through them, so those are not good options for sealing a chest wound. My recommendation is to get some basic medical training (CPR/first-aid, TCCC, etc.) and become comfortable with your equipment. Carrying medical supplies but not having any training on how to use those items is pointless. But at the end of the day, I guess you could just put your hand over the wound until you have the proper equipment (you're not going to get a good seal, however)....good luck if there is an entry AND exit wound, though lol

Great video! Espcially outlining that preassure is the essential for the outcome, whereas the hemostatic effect is "just" a supportive factor. However I think it's a bit misleading to show the CELOX granulate when talking about the powder based hemostatic agents that caused burns, as this has never been an issue with Celox but rather (don't quote me on this) with zeolite based agents only.

QuikClot Combat Gauze and Celox both have five-year shelf lives, and the individual products come marked with appropriate expiration dates. What if you use the powder, the roller gauze, then put a tourniquet for the same wound? Wounds are obviously random and unexpected. Getting the very best or very worst for every wound isn't good. At least with the 3 stuff I've said, you can upgrade the treatment or keep it basic. At basically $2 each for the Bleedstop powder it's a lot better than that $41 one in the air tight package. I have Combat Trauma Dressing that don't have any powder. Having it in powder packets means you can use it with other stuff too instead of it being limited to just that gauze. I just made my kit from having no idea 1-2 weeks ago.
@PewLand Жыл бұрын
I've been having the same thoughts. It seems to be a much more economical way to achieve the same goal.
@antdx316 Жыл бұрын
@@PewLand The scarcity of supplies may be the reason why people tend to be extravagant and take five items when they only need one. If people were aware that more items could be added, they might demand more, leading to insufficient supplies for others. Having more supplies should improve treatment quality as it provides better protection. If people were more cautious and aware, accidents could be prevented. This means that accidents should only occur in unforeseen circumstances, and not in areas where they usually happen.

Why people sell ifak's with both hemostatic and regular gauze? it wouldn't be better just to have hemostatic ones?

Studies show that properly packed with non-hemostatic gauze a wound will still clot. Hemostatic gauze does not increase clotting rates, and still needs to be applied with pressure for a few minutes to induce clotting. So does standard gauze. Standard gauze is cheaper, however.

Why did you say "misinformation"? I am sick of that word. That is a biden-word. So I will start listening when I hear the words, "incorrect information". So now, I have lost interest.

I'm a new sub and I'm loving your videos so far! I've been certified as an EMT for a little over a year now but haven't joined an agency yet. Your videos make me very excited to start this semester! (I'm a college student)

For years I heard not use tampons, just about as long as to use them. This is the first time I've heard why not use them. They are designed to absorb blood, not to cause clotting. Hence, the thick weave and women not clotting every month. Thanks for the info.
@lorknozzel114 жыл бұрын
not to mention that tampons are only meant to absorb about a teaspoon's worth of blood. maybe a bit more. and they do not expand all that much to be useful in applying pressure. if they did, most women would find them very uncomfortable to use.

Thanks. This video helped clear things up for me -- after taking a tactical-medical-trauma class. I know more about how to purchase and equip my kit now.

Are you on a small budget?: Buy regular gauze Do you have the money to spend?: hemostatic gauze will not hurt. But do not forget that Israeli Bandages and Tourniquets exist. Using a trauma Bandage (Israeli) in conjunction with hemostatic packing gauze is my go to. It's more expensive but being able to keep constant pressure passively is an invaluable luxury. One person is unable to keep pressure on multiple wounds let alone multiple people.

I deal with a lot of open head wounds, and have found that I can cut a couple layers of hemo gauze and apply that directly to the wound, and then cover it with regular gauze, and it works quite well. Stretches my hemo gauze further, because, yes, it is expensive.

Are these cases where the subjects are being seen at the hospital? I only mention this because if you’re using the same package of hemostatic gauze in more than one application the sterility was already compromised upon a opening and the subject would likely need antibiotics

I thought hemostatics were contraindicated for head wounds

$20/$30 seems like a lot when you don't have a cut/wound. I had to use the QuickClot after cutting my thumb. It worked great, so I'm going to buy some more of it.
@Calchick73 жыл бұрын
used quickclot powder? or was it a gauze with it in it?

Good for you. I don't think he has ever used it personally. It stops bleeding.

I've always wondered (in real world situations) if Celox could cause fatal allergic situations to those allergic to crustaceans? Literature stated that there was no known reactions. BUT. My wife once had a reaction because I was cooking frozen fried shrimp while also cooking french fries in the same pan in the oven. They never touched each other, but the not breathing started to happen when she ate some fries, so would placing their crushed shells onto and into an open wound (direct contact with bloodstream) cause unwelcomed reactions? I understand that its not just "crushed shells" but proteins are still very mysterious.

With the traditionally produced ones, i can sure see that. Not so much with the synthetic Chitosan, but i am not a doctor.

Is one of the advantages of hemostatic gauze, that pressure is applied for a reduced time?
@wacojones80623 жыл бұрын
Hold pressure until the bleeding stops it may be up to two minutes faster if properly done and you are not interrupted if the tactical situation dictates taking defensive actions to protect yourself and your patient. None junctional wound Tourniquet of arm or leg if under fire High and tight try to get a pressure bandage on tight within 3 minutes and release the tourniquet and check for distal pulse if no pulse loosen the bandage and recheck for pulse if bleeding is still present tourniquet 3 to 4 inches above the wound not on a joint and if needed pack and rebandage.

Newbie here: I am somewhat confused. I've heard that it's not always necessary to use hemostatic (I would ASSUME you'd always use it if it's obviously severe bleeding) if you have both. If you have both at ready, do you always pick the hemostatic? If not, what do you do if the non-hemostatic gauze doesn't stop the bleeding? Is it just because It's a bad packing job? I'd assume you don't pull the gauze out and re-pack with hemostatic. Are you kind of shit-out-of-luck? I know this is a lot of what-if's but, I like to know as much as possible so I try to not be caught with my pants down. Thanks.

I'd say if the wound is deep enough that it requires packing to stop the gushing, and you have hemostatic gauze, go in with that first. Then if more gauze is required to fill the wound, use regular gauze and top it off with an Israeli type compression bandage.
