Med student here. Another point I’d like to add is that many of these mortality studies are done on *hospitalized* patients. These are sick patients that could plummet at any moment, becoming hypoglycemic or hypotensive. To prevent a scenario where that might happen, we tend to keep patients on the slightly higher end (systolic 140s, glucose 140s). This way, if their pressure or glucose drops a bit, they have some wiggle room. Hypoglycemia and hypotension are more dangerous than hyperglycemia and hypertension (in the short term, such as during a hospital stay, because you could pass out and hit your head, or get a kidney injury from reduced perfusion). Patients are generally advised to resume their prescribed home med regimen once discharged.

My father in law passed out and hit his head on a walk. Way over medicated for BP. He had multiple falls before they lowered his dose and it fixed it but by that time he had lost a ton of mobility and muscle mass due to being scared to walk at all.

Until they correct for people who don't drink because of health problems. It's the same for sleep, and people will argue that you can get "too much sleep". If you get 8 hours every night and still have trouble getting up and get sleepy during the day, sleeping an extra hour is going to improve your health, not harm it. Of course sleeping a lot can indicate a health problem and there are health problems that can allow you to get too much sleep, but if you are healthy, then when you get enough sleep you wake up.

Another thing these studies don't adjust for is people who got healthier numbers later in life precisely due to the fact that they started having problems. E.g., people start having chest pains, so they go to a doctor, discover they have heart disease, and go on cholesterol lowering medication. So then their cholesterol numbers are great, but it doesn't change the fact that they have heart disease and are therefore more likely to die from heart disease.

What you need to adjust for is age. It is a fact that the average metrics tend to be higher for all those who survive to old age, independent of explanatory factors. Other doctors have pointed this fact out. That is, what is normal depends on age, as most clinical studies indicate.

I'm a 62 year old man with high cholesterol know matter how I eat or exercise it's still high I've been taking Crestor for 7 years now so far so good. I was one of those preaching high cholesterol is healthy because one of those conspiracy based "doctors" had taught me that our brains are mainly made of cholesterol and so we needed it to be high and that the low cholesterol diet is an idea from sociopaths who want to get people sick for money. Fortunately, since the pandemic I have been listening to your content and honestly your videos have helped me understand nutrition, and nutritional research better, so now I also do some canola oil, have reduced the consumption of eggs weekly, added more veggies, less salt, less saturated fats. Also I have stopped telling people these claims I have no certainty nor proof of. A lot of it is thanks to your work, so keep it up! Thank you for doing what you do. Cheers from Mexico.

I am on the same page. I also went down the low carb high animal fat rabbit hole. While my a1/c went to low to mid 5’s, my lipids doubled into not so good range. I now am eating more of a Mediterranean style diet, less animal fats and more fish and veggies and adding more fiber foods to help lower my lipid levels. I don’t even want to get into the “our bodies don’t need fiber” misinformation out there. I have my labs in April of next year, I hope things have changed for the better.

eggs do not affect cholesterol with regards to heart disease in everyone I have higher LDL and eggs do not affect it at all - it makes no difference if I eat them or not - as I've tested myself, in fact my cholesterol was higher without eggs replacing eggs with other stuff, like yoghurt with nuts/seeds for breakfast. So no eggs for six weeks, LDL higher. there's a study that showed "People who ate 12 fortified eggs per week had similar cholesterol levels after four months as people who ate fewer than two eggs per week". some people are hyper cholesterol absorbers, but most are not. guess it all depends. I think something worth noting is that Dr. Carvalho has both an MD and a PhD in biology. While medical doctors will learn many of the same skills regarding reading and interpreting medical research, Dr. Carvalho likely has a lot more specialized experience in this area that not all medical doctors will have. This is why his input is so valuable, since he has not only the ability to explain things from a medical perspective, but he has a wealth of knowledge about research design itself.

Interesting. I actually had a dream last night that I was explaining how statins work to my husband's cardiologist. He was wanting to know why he wouldn't take one even tho his triglycerides & total cholesterol were 'high' tho LDL was low. Not a dream but in reality my husband has just had a CCTA that proved that he has halted & begun to reverse the badly calcified arteries discovered 3 years ago. Through diet. WFPB no oil with Dr. Caldwell Esselstyn's program. He had previously discontinued the meds he was on & the cardiologist was stumped as to why he could not put him back in the cath lab. He kept insisting that CAD is progressive... in other words, you're supposed to be getting worse.

Great video, thanks. It's the same argument with alcohol consumption. Correlations show that people who have a few drinks are healthier than people who have none. When you take out the people who have quit drinking because they are sick, alcohol doesn't have a protective effect.

I'm also glad you mention the different LDL fractions briefly at the end of the video. For us to discuss "cholesterol" is a very coarse and imprecise thing. We're really talking about lipoproteins. My follow up questions as a patient would be: "Why is my cholesterol so high?"; "Is this high level a response to or consequence of some other stressor, adaptation or disease state in the body?"; and "Is blindly seeking to reduce cholesterol levels without addressing the cause of the high levels a good thing or could it cause more harm than good?"

Blindly seeking to reduce cholesterol has decreased mortality in RCTs time and again for 40 years. So no, it's not causing more harm than good. Maybe drilling down on why cholesterol is high to begin with might be a better effort, but not doing so is certainly not causing more harm than not.

I am trying to find the truth though, I think a total cholesterol just above the limit is still worth trying to get down with exercise, lo carb, lo fat and lo sugar with extra oily fish is better to try before statins. Also not all LDL is bad, depends on ApoB and how oxidised the molecules are, also dieting and illness can raise cholesterol temporarily.

The main point of the video is trying to tell you to stop blindly believing what a likable or authority figure tells you without understanding the evidence. Looking at charts for hazard ratio - all cause mortality and total cholesterol, the sweet spot for healthy cholesterol is somewhere between 150 and 250 (bottom of curve). The charts definitely show spiking , unhealthy results above this level. Do you have data, studies for total cholesterol in the 200 to 250 range. Which had been considered normal for decades as some of your older viewers may recall. Appreciate you sharing information keeping a balance of information.

I can't imagine keeping BP high is good. My long time boyfriend died in February of an aortic aneurysm (with dissection). He had uncontrolled high BP at one time 190/140 with my home monitor.. Don't even know how long it had been this high. I pleaded with him to see a doc but he was reluctant, said he always felt fine. Finally he did and was taking a couple of different meds for a few months,it came down some but told him to see his doc again and have a cardiac workup. Aneurysm started to bleed and ruptured, he was rushed into surgery but surgeon said trying to repair the artery was like trying to sew wet tissue. He died on the operating table, only 50 yrs old.

On your glucose vs cholesterol argument...the body is quick to get the glucose out of the blood stream, but it doesn't do the same with cholesterol. That would be somewhat of an indication that glucose in the bloodstream isn't good, but cholesterol isn't necessarily bad. Edit: What are the downsides/dangers of high cholesterol if I dont have any plaque?

hi, thanks for your question, if everything's working well, glucose is maintained in a homeostatic range, but in diabetics & prediabetics glucose is abnormally high 24/7, potentially causing problems. Similarly, the body attempts to remove lipid-carrying lipoproteins from circulation if there's too many of them, but many factors including genetics and lifestyle affect this process Having no detectable plaque (e.g. by CTA) is a good sign, although the question is time of exposure. No plaque seen in imaging in a 30 year old is good news but not as reassuring as no plaque in a 80yo which indicates very low individual susceptibility

I found out my LDL was 214 in August. So I did what everyone does these days. I went to KZbin. Lo and behold, I was bombarded by influencers telling me that it is ok to have elevated LDL as long as I follow a low-carb diet. For the first few weeks, I did exactly that. And because I'm also prediabetic, the algorithm led me to Dr. Mario Kratz's channel, and from there, it led me to you. I have since changed my diet to mostly WFPB. I'm due to check my blood again in January, so hopefully, the numbers are a little better. It saddens me how much misinformation is out there!

My cholesterol was also high 200+ at one point. Was taking low dose Zetia and it dropped some to 187 but stayed around that level. I do not like statins, tend to have an adverse reaction to them except for low dose Zetia. Then went on strictly WFPB diet, eliminated a lot of sugar/carbs and use very little oil/fats. Had bloodwork done last week cholesterol had dropped dramatically to 107. All other of my blood work was perfect 73 now wish I had done this earlier in life, fortunately, I'm in excellent health, not frail...lol, ideal weight for my height, BP perfect. Hope to stay that way as I get older.

what's your weight? to me a mostly WFPB diet improved my LDL (which wasn't that high, just under 100 mg/dL) but I needed to lose weight for it to improve further after I reached a BMI of 22.5 it got to 73.5 and I expect it to go even lower

THIS content is nutrition made simple! Best video you've done in a while, for my money. To be fair, I don't think your whole channel should focus on debunking health misinformation, but this kind of stuff does have a way of getting to the heart of the matter, at least in practical terms. And debunking it then builds clarity around those important aspects. Just my 2 cents.

I had an entire course on how to correctly represent the data on diagrams, how to read and how to spot when the data is being misrepresented or is presented in a way that it easily can be misunderstood. Still, I am careful when looking at some papers regarding anything related to the field I am not working in and nutrition is one of them. This is why it is important for educated people, people who are experts in their field, to read the research papers, explain the data to people and explain how to understand what is being shown. Thank you.

I calmed my gut inflammation following the protocol you did for your mom, who can tell you to avoid vegetables to heal gut 🙌🏼 I did white rice, simple carbs and potatoes, and avoided vegetables for months, now I can eat most foods with no reaction ✅

The late Dr. Mike Cheraskin, one of my favorite CME providers, would give you very high marks. He stressed the kind of rigorous examination of data and not forgetting small scale studies--along with reminding that true homeostasis was the best benchmark for a guide. 55 years later I still remember his saying about a claim that the lower BP the better " So by that logic 0/0 would be perfect!"

I sort of understood relative healthfulness when I was a young child and heard of a baby who became blind due to high oxygen levels after birth. Although I had always heard of oxygen in the context of it being vital and good, hearing that in excess it could be devastating set me up to understand that too much of even good things could be bad.

For my understanding, can you elaborate on why my cholesterol is going up a bit when eating healthier now? Almost no sugar, eat more vegtables than ever before, eats healthy fats, only eat between 11AM and 6 PM. All other markers seem fine. Would be interesting to understand.

Great video but I am still confused, I have high cholesterol but my blood pressure is in the optimal range, I am 62years old and my blood pressure is the same as my 23 year old healthy son

They are not directly related. You can be fit and have a normal bp, but still high cholesterol. Cholesterol is mainly diet related.

I wonder if you could explain why im fit as a fiddle nearly 60 smoked for nearly 50 years , drink a decent amount , excellent blood results and dont even have a smokers cough no illness ,or take any meds whatsoever ? Maybe its something to do with being fit and with spot on bmi and eat great food , and a very exercised mind , and very well informed .
