Just got this procedure done. Opted for no numbness, and honestly I was fine with the insertion, a little sharp pain, but once it got past the inside of my nose it was okay. The next step is feeling it at the back of your throat, this was weird, and I kept trying to swallow it down, it wasn't uncomfortable though. I then asked could I have a cup with a straw to swallow it down, this helped loads. Little sips, and then it was in place. I spent all last night so nervous over the comments from people, but it is not that bad. Relax, focus on a point, and focus on breathing in and out, it really is mind over matter.

I got mine done a month ago. It all comes down to whether you can be a big boy or girl. Relax, it doesn’t hurt. Is it comfortable? No, it isn’t. They first numb your nose with a spray to get passed the sensitive part when they are sliding the catheter through it. They will give you water to sip on with a straw to help feed the catheter down your throat to your stomach. Once it’s in place they will have you lay down in a reclining position most likely. (That’s how I was.) You can feel the catheter in your throat it isn’t comfortable but it doesn’t hurt you just need to stay calm and listen to the nurse or technician that is performing the test on you. Take some satisfaction in the worst part of the test is over. They will then begin the test with giving you some water to swallow. After that, they test for about 15-20 seconds. They will remind you not to swallow during that time until the next set of water they give you to swallow again. This will be done about ten times with about 15-20 second intervals. After the last one you are done and they will gently take the catheter out. You’re there for about 15 min it really isn’t that bad you just need to breathe and stay calm. Good luck to whoever did what I did in looking for a video of the test before getting it done lol. You’ll be alright.

**UPDATE** I was diagnosed with what I pretty much knew already of what it was going to be . Achalasia occurs when nerves in the esophagus become damaged. As a result, the esophagus becomes paralyzed and dilated over time and eventually loses the ability to squeeze food down into the stomach. Food then collects in the esophagus, sometimes fermenting and washing back up into the mouth.

@@chaithdridi2718Thank you, I appreciate it. I met with another doctor this past Thursday. He told me once you have Achalasia you always have it there is no cure to reverse it. What they can do to help with my symptoms and struggles everyday is a surgery that cuts the esophagus valve (sphincter) so it will stay open instead of closed which is what mine is doing. I get it done February 8th. He said because of my age this procedure benefits the best for me long term. The other procedures they’ve also done is with a balloon expanding the esophagus so it stays open and a Botox injection but these ways only last for so long. He has seen significant improvement with his past patients doing the procedure I’m going to get. We’ll see what happens. I’ll give an update a few weeks after I had the procedure to let everyone know how it went and if it worked. I was told by this doctor it’s an autoimmune disorder where the cells in the esophagus die causing the esophagus to not work right anymore due to a viral infection or something in the past.

So I had the surgery one week ago and even though I am still healing from it I can feel a huge difference in my swallowing. My doctor had me on a clear liquid diet the same night I had the surgery (which took about two hours) he told me they had to do a lot of cutting around the valve. The next two days, I was on a liquid diet (smoothies, shakes, cream soups etc) then a soft diet for three days (scrambled eggs, mashed potatoes, overcooked pasta) I am now eating regular food again but now over doing it. Taking my time eating slowly not huge bites but so far it has been great no issues with the esophagus sphincter. Still have some throat and mouth irritation from the breathing tube etc. and I had pain for the first few days it’s healing so hopefully Everything will be good soon. I lost more weight during this past week cause the diet hasn’t been the best but I am eating better now and hopefully I start putting weight on again now. Good luck to everyone who is going through this you are not alone and the procedure I had done was the POEM procedure. Per Oral endoscopic myotomy. Good luck everyone ❤️

This test was performed on me in April, and in all honesty it was NOT that bad whatsoever. I didn't gag, and it really wasn't any pained involved. It was a little sore in the nostril that the catheter was inserted in after the procedure. But other then that it was perfectly fine. I have been meaning to share my experience on here for quite sometime now.

Just had this done today in Canada. Well was supposed to have it done. The nurse used no topical anesthetic at all. Took three tries before she got the catheter in and I lasted about 60 seconds before telling her to get the damn thing out NOW. I've been through a lot of different painful things ranging from broken bones and cuts to migraines, torn ligaments, bladder catheters and various recoveries from surgery and I have NEVER experienced anything even close to as painful and uncomfortable as that was. I've had ESS surgery along with a septoplasty so trust me when I say I've had many different things passed through my nasal passages and know what I'm talking about. This was brutal and I am launching a formal complaint with my health region in the morning.

I don't under stand why this can't be done through you mouth instead of your nose. I had it done through my nose and it was horrifying. I have PTSD from it. No way ever again. I cried screamed choked gagged, felt like I was going to pass out. This torture test should be outlawed! I didn't even have any numbing beforehand, which I doubt would of helped much. I was fine once the cable was fully inserted, but until then, it was the most brutal test I have EVER been through! I just had mine done yesterday it was so uncomfortable. It didn’t want to enter my left nostril I have a ton of allergies so she went onto my right one. my eyes were crying non stop from the moment the wire hit my throat I was saying I couldn’t do it and I grabbed the techs hand and he was so nice to let me grab his hand I was so scared I gagged like 6 times. The nurse was putting the wire in my throat and they told me the best thing to do is follow the instructions when they tell you to swallow open your mouth right after!! and breathe and not to swallow until instructed again because they would have to redo it. It took them 7 minutes to finish I was just telling myself to zone out because it’s very uncomfortable and my heart rate was very high once the nurse tapped the wire she held my hand while the tech put the salt water in my mouth. Once they removed it I thanked them for holding my hand because my anxiety got to me. I bled a bit after from my nose and had a very light fever last night. Although they did say “ if you get a fever go to the Er” my body is very sensitive so I just waited it off to see if it progressed. I feel better just slight sore throat.

Patients deserve complete and honest information before they agree to one of these ‘tests’. They typically get some sanitized nonsense that glosses over the worst parts of the test and acts like it is nothing. These patients are then shocked and horrified when the test turns out to be miserable or painful. I had some idiot try to give me the soft sell for a transesophageal echocardiogram. Unfortunately for him I am a retired nurse anesthetist and knew EXACTLY what was involved. I almost tore his head off. He finally admits “It’s not a very pleasant procedure”. No S, Sherlock. How about being honest and letting patients know what they are in for?

What entailed was literally the most horrible thing I've ever had done, I can only compare it to repeatedly vomiting over and over, except in a room of people watching you. I left feeling humiliated and crying. If I'd have just been TOLD it was going to be really horrible, at least I'd have been prepared!! It's not as if it will magically change the procedure if they say it will be fine, so i don't understand it.

I just tried having this done. I've had endoscopies done. I've had a scope placed into my vocal cords. Both of those were not too bad. But this was horrible. The nurse was having difficulty placing it so I asked for numbing spray. They only had gel. I placed it into my nostril and sucked it down. Just had a little trouble swallowing it. Then the last try she got it part of the way down and kept telling me to swallow, swallow, swallow. I was trying to swallow as hard as I could. She stopped coaching me so I tapped her hand to pull it out then I coughed and gagged really hard and spit into a barf bag. I dont think she was trained very well in the procedure. There were 2 other nurses assisting. They should have been more helpful with the process by talking me through but they were silent. I asked for a different test. They said this was the best one. Maybe the doctor should be the one doing it or someone better trained. I felt like I was being choked. They didnt give me water and a straw like in the video. So disappointed. What are my options now?

someone i know had this done recently. they said nurse sprayed a decongestant and anaesthesia lidocaine, and gave them water cup with a straw. They looked straight ahead, and the nurse slowly put in the catheter measuring tube, while the person swallowed water which also swallowed the tube. When it was in, when the person ate a meal, and they chewed food a lot until it was liquid, which was less painful when they swallowed. Rotating between max doses of pain meds made the throat pain bearable. For a bit the catheter tube was laying in a bad spot in the nose and made the person tear up and have a headache, but they solved it by very slightly testing which way to move the catheter back out of the way. When it was removed, they breathed out, and it was a very easy and painless process because they nurse was also very skilled.

I’m having this done on 3/14, and I’m very nervous, especially after reading the comments. My biggest fear, believe it or not, is mostly the numbing spray. I’ve been having difficulty swallowing, which causes me to constantly choke, so I’m afraid about what will happen if they numb my throat. Will I be able to breathe, and if I refuse the spray, will that make the procedure extremely painful without it? I just want this to be over. It feels like I’m having constant esophageal spasms, and it makes breathing very difficult as well. Other than Gerd, the doctors haven’t been able to find anything wrong and can’t tell me why I’m having these esophageal problems. I’m praying that this test finally gives me a solution, and hopefully it’s nothing that’s incurable.

How did it go? I've had three manometries, two of which without the spray, and on the third attempt they had to remove it and start over because of my esophageal issues 😑. I also don't like the spray, it feel like your throat is swollen but it isn't really. But without isn't too painful either. A little like a Covid test, the pain is gone before you have a chance to complain about it. 😂

I focused on something I like and kept breathing heavily through my nose and mouth continuously. Sore throat after the procedure but I was in an out in exactly 15 minutes . There is no pain absolutely but the tube in your throat makes you uncomfortable, try not to think about it and focus on anything you can and again keep breathing , very important. And yeah I declined to lay down and did the rest sitting up ( I felt it was easier to just sit up and meditate )

Ashocka, that test is carried out first to test the way the muscles and sphincter work in the oesophagus, the esophageal motility study or high resolution impedance manometry study. Depending on how well you tolerate it and if you can manage to not swallow in between the water they syringe, if you swallow before the time they have to wait again before asking you to try again, will reflect on how long the test takes and therefore how long tube is in place. (Minutes, nowhere near an hour) The test if required done after this procedure is the impedance_ph study (either done still on ppi or done off ppi as required by dr.), this is also carried out the same way but using a smaller catheter, it remains in place for 24hours. It is attached to a digital recording device which you have to press buttons to correspond with the symptoms you experienced and when you lie down and become upright again. Hope this helps and good luck for your test if you haven't already had it performed.

I had three manometries and the third time they had to remove the canula and start over, because of my esophageal problems. I've had it done both with and without the spray. For me, initially it felt like a Covid test the first time, when they hit the back of my nose, a little painful but only for a second. The next two times it didn't hurt, the cable went through without a hitch. The last time they used numbing spray and I didn't feel a thing, but make sure they spray your throat and not your tongue. It feels like your throat is swollen, but it isn't. When they reach your throat, it can trigger a gag reflex. Of course it's very personal how strong that is. I recommend you use the spray if it's strong. If you have food in your esophagus, it's more difficult to place the canula. On top of that, you will puke all over yourself so make sure they cover your clothes and ask for something to puke in. If you have severe achalasia, I highly recommend you stop eating solid foods two or three days in advance and only drink clear liquids 24 hours before, to avoid this situation. With achalasia it can be difficult for them to know if the canula is in place. A trick my doctor told me is that ask you to blow out all the air, so they can see your diaphragm moves. If you're unsure it's placed correctly (eg if the person doing the exam seems unsure or has a lot of trouble positioning it, suggest they have a doctor come in to make sure. If you're nervous, tell them. If you have a preferred side you want them to use, that's not strange, tell them. Tell them how you feel, ask them to (not) play music or talk to you. In my experience health care workers really want to help and make it easier for you. If you have any other concerns or health issues that make this difficult for you, tell them upfront so you can come up with a plan. Removing the canula in my experience was not painful at all, they do it quickly in the video and that's normal. At that point, you're going to be glad they're taking it out and they can't work fast enough. 😂 What helped me get through: focus on why I needed it done, to help me get better care because it's a really important diagnostic tool. Focus on how I'd be outside again in fifteen minutes, or only X sips of water - I can survive 15 minutes of discomfort. Focus on my breathing.

The numbing spray helps, they need to spray liberally though. Sometimes they hit your tongue and it doesn't hit the back of your throat so it doesn't help. Other than that, maybe practice with breathing exercises and meditation so you can lean to focus and stay calm. That helps me, just focus on my breathing. Also depending on what they think you have, maybe stop eating solid foods a few days in advance. If there's food in your esophagus it's more difficult to get the canule in place and you'll puke your guts out. 😬

I've got a 24-hour one in now, and it's not too bad. The large tube they do the first test is the worst part. Put it this way...if this test tells them why I get indigestion so bad, it's a small price to pay. I didn't get any numbing spray either....it wasn't offered.

It doesn't hurt when you swallow it but as soon as it goes down into your esophagus it hurt your throat and it's hard not to gag, throw up and dry heave. "Will be removed GENTLY" *proceeds to YANK it tf out* Yeah this shit is NOT fun, I have VERY thin nostrils and she practically had to SHOVE it into my nose to get it to go in because the numbing gel didn't do shit and I almost gagged when I had it done to me. Every single time you swallow it shifts and slams into your uvula. I have a constricting throat as well, very thin and doesn't shift well in response (one symptom I have from my hiatal hernia is difficulty swallowing and trouble with eating/drinking fast) there was NO way I could do "rapid fire swallowing" back to back like she was asking. I kept swallowing to keep from gagging and teared up, I was panic-breathing too. At the end, the assistant tugged it right out and it created a searing hot burning pain inside of my nose and throat. I had to ice my nose and gargle ice water when I got home. It hurt so unbelievably bad because I could feel the tube shifting in my neck when I would swallow. I'm also emetophobic so this test was naturally horrific, but after doing this test I'm really dreading the 24-hour PH test.
