Everyone that has high SHBG needs to try the following before getting on TRT. I fixed these two things and it solved my high SHBG issue (124) 1. You’re overtraining. No reason for a natural to train more than an hour 4-5 days per week. 2. Your fats and or carbs are too low. A 45/30/25 c,f,p is the optimal macro ratio for high free testosterone.

I've got SIBO/SIFO gut problems. I can't mess with carbs unless I've gone for extended perioids w/o them and minimal flares. Even then I can only have carbs for short periods and in small quantities. Sugars off any kind are completely out of the question. I had some peaches a month ago and they blew my gut right up. This whole thing is a pain in the arse and I think many of us have been hit with bioweapons or toxins in our food creating this vast panoply of disorders. It's ridiculous to see this sort of juggling just to function, even at abysmal levels. Good luck to those who have the fortune to be able to enjoy carbs.

@@LegalTyranny I recently discovered that in histamine intolerant, so I am extremely limited now. Have you tried making gluten free oats, basmati rice, and sweet potato your main carb base? Those don’t irritate my stomach at all.

@@philmcrackinnow What I have as a diagnosis is SIBO/SIFO so I literally can't do most carbs. Fibers of a certain type are okay. There's a dietary plan called low FODMAP which is supposed to be low fermentation foods that helps prevent the infection from thriving and bloating my gut when I eat but all of this stuff causes nutrient deficiencies when you're on it long term. I'm looking at copper and cobalt deficiencies that may have been caused from 2 months of doxycyycline anti-biotics after I got lyme disease from a tick bite. Supplementing and grass fed beef liver seem to be the way to get those 2 elements back up which help the gut and kill bacteria. I had histamine problems for about 3 years. You should google histamine foods and histamine releasing foods. Many probiotics make this worse. Phage complete has probiotics that break down histamines so look that one up. Also look up "the gut health protocol" book for $20 by john Heron. I was a complete mess before using that as a guide. I still have SIBO but the other crap like histamine issues are at least 4-5 years behind me now. Also look into Larch tree fiber. That one is great for reducing histamine producing bacteria while promoting good bacteria growth. Swanson has the best prices on amazon but the product doesn't stay in stock due to demand. You might be able to find lists of histamine free foods and non-histamine releasing foods (some food has actual histamines in it, and other cause your body to release histamines). You might want to look into whether or not you have straight up "amine" sensitivities because that was my case.Histamine, any supplements with "amines" like vitamine B2 in the Bentiofolamine form. Also look into phenol and salicylate sensitivity. If those are an issue epsom salt soaks for an hour each day will eventually help along with serious dietary restrictions. Sorry for the wall of text verbal diarrhea but this whole area is such burden cropping up for so many right now and it can literally make your life hell not being able to eat any thing with out reacting in some way. Take a serious look at the gut health protocol book. I'm a former published brain research scientist and the guy that researched and wrote that did graduate level type research for it.

Very good video. You covered more info in this 7 minute clip than the 3 specialist I’ve seen for my high shbg (110😳). Trying for trt now even though my free t is within range. 230 on a scale of 196-636. Unacceptable for a very active 33 year old first responder. The mental acuity just isn’t there with that free t being in the gutter.

My total was 890, my SBGH was 96 and my free was 9. I went on TRT (at 64 - now 68) and raised my free T to 25-30. I saw significant improvement across the board in terms of added muscle, lower fat and better metabolic, lipid numbers and, of course, libido. That said, I wish the doctor - who tends to put everyone on TRT which he marks up from the compounding pharmacy - had told me that my Keto diet (lost 80 lbs 7 years ago and kept it off staying low carb) had dramatically increased my SHBG through the lowering of insulin from 20 to 2. It turns out that SHBG has an inverted relationship with free T. People with high T but low free T, should access whether or not low insulin might be driving down free T. I may have decided to do TRT anyway given low insulin levels are critical to achieving good health outcomes and that raising T through TRT is the best way to mitigate its impact on free T, but the doctor should have told me the cause of my free T and let me make an informed decision. Danny, thanks for all the work that you, Steven, Gill and Jordan do. You guys got me off of AIs, convinced me to do more frequently dose (I do every 3 days which seems to work good for those with high SHBG) and to stop taking a DHT suppressor. Keep up the great work!

@@andinov your estradiol is very low because your free T is low - all because your SHBG is high. Keto will only make it worse because it will cause your insulin to drop which will keep your free T low. That’s created a dilemma because keto is a good diet for that very reason. If you retest and your free T is less than 1% of your total T, I would consider TRT to lower your SHBG, which will increase your free T and estradiol while being able to stay on Keto.

@@stephenlatham9172 I agree - the higher the SHGB, the less frequent the administration. However, once every 3 days is ok in order to keep your levels more even and thus a lower hematocrit rate (less thick blood) and a lower estradiol. I have listen to these guys on this subject a few times and their position is that with higher SHBG you will be a high responder and thus never need to pin daily and that you should start out once a week and eventually go to twice a week. The key her is to watch how your hematocrit levels- mine went from 54 to 46 by going every three days and I have a high SHBG. But, if you want to do weekly and you have a high SHBG, you will probably do fine on once a week. I like the more steady levels achieved at every 3 days. If I had very low SHBG, I would probably pin every day with an insulin syringe.

I'm mid-30s battling this problem with SHBG. Should I stop intermittent fasting? I find that I lose weight and feel good when I fast...but I wonder now if it's only going to make my SHBG that much higher. I keep taking an estrogen inhibitor...but I wonder how much help it's really giving me.

So your advice is to go on TRT? I've tried boron, P-5-P, various foods, upped calories, increased fat / carbs, and lowered training volume, all to no avail.

@@TheDannyBossaPodcast Does magnesium free up more testosterone? I read that online but don't know if thats true... As lots of people are deficient in magnesium, maybe suplementing that may help a little bit?

@@RickAstleyLetMeDown virtually everyone is deficient in magnesium and it is responsible for hundreds of thousands of processes in the body. Supplementing is always a good idea. Never heard anything about it freeing up more testosterone though.

Thanks for the video Danny... I'm the opposite on my SHBG. My SHBG is 12.9 - total T is 1330 - Free T is 47 - Estradiol is 63.2 feel pretty good with no sides, injecting 4x a week @ 180 mg Test C

Watched a lot of these videos and no one as of yet tells you the range. I got three blood test, dates 4/6/23 SHBG 68.9 - Range 19.3-76.4 One lab. Second lab: 4/20/23 SHBG 57.2- *Range 22-77; Third lab: 5/15/23 69.5- *Range 21.4-67.8. So the third lab shows out of range according to the range all three were out of range if I used the thrird lab range. Anyone know what the true range is?

I just received my result after the 2 first month of trt 150 mg/week My testo is 1159, estradial :53,87(high?) and SHBG 34, 3. i was 2 month ago Testo 605. estradial 21,2 and SHBG. 49,1 what do you think about my result, shoudl i lower the trt dose? and is the estradial too high?

With the help of my naturopath I was able to bring my SHBG down from 127 to 76 over 2 months using 500mg of TUDCA twice per day.

My SHBG has always averaged in the low to mid 70s and I tried TUDCA for about 4 weeks and my SHBG didn't budge. I was taking 500mg per day and I started feeling really dizzy taking it so not sure really what was going on so I stopped. Do you mind sharing what brand of TUDCA you used? Thanks!

@@zdaher Great thanks! Yes, I was using the Double Wood brand as well. I wonder if me getting dizzy was just a coincidence. Maybe I will try it again. I'm assuming you haven't had any side effects? It definitely gets the bile flowing which in turns helps gallbladder, liver, and bowel function.

@@Total_Body_Fitness_USA No side effects to date & I'm using double the dose you were using. It also brought my ALT down from 50 to 29. It is very expensive here in Australia so I'm not sure if I will keep using it full time after my next bloods but will assess and move forward from there. So far only positive things have happened.

SHBG is the reason muscle growth to a noticeable degree is essentially impossible, It’s role among other functions is to improve your survivability by resisting development of expensive to maintain body tissues. You could have 1000 mg of test and your body will just catch up with proportionate levels of SHBG. The main role of PEDs is to outnumber your SHBG levels to allow for your body to be anabolic enough and not resist noticeable muscle growth, if SHBG elimination was a possibility you could also have “low t” and grow muscles like a hero. Our bodies are “traumatized” by tens of thousands of years living under scarce and poor and harsh conditions and are not even remotely caught up with the lifestyle we are carrying today, so our system could only give a fk about any natural visually noticeable muscle growth when it is programmed to make sure to synthesize as little protein as possible while storing reserves aka fats so as to avoid waste. In short our biology is literally retarded and haven’t quite “updated” to fit our modern life style.

My doctor started me on 200 mg of test cyp once a month and I felt horrible. Based on what I learned here from Danny I talked him into 100 mg per week and I feel so much better. There are lots of doctors that don’t have a clue about TRT. I get my bloods done in 8 weeks and based on how I feel they should be good.

now come on, Bayer designed that two week protocol 50 years ago based solely on keeping total testosteron "in range". Why on earth would they bother to do proper research with updated knowledge and properly designed studies? You're asking them to spend money to sell something that sells itself? So Kaiser that is a major health insurance in Ca. will as a rule not measure free testosterone or shbg. As a person that has free Test around 8 and total of 800 I have begged them to measure SHBG, Free and everything else, but they say I am fine if not high already. so no dice.

So I now listened further and the example you gave of a doctor driving up the Total Test. to raise the Free test, is exactly what happened to me with devastating results. The doctor gave me 1 cc every week/10 days, and my Free test got to somewhere around the bottom of the normal, but in doing so the Total Test became 2400. within 4-5 months my balls were gone , my nipples were super sensitive, my hair was falling out, and worst of all I became impotent. That was 12 years ago, and it (sex drive or ability) never came back. Poor wife.

I don't think that SHBG is an enzyme; it's a glycoprotein. So, makes it hard to accept the rest of what you say!

I read somewhere that iron overload can cause the liver to produce too much SHBG, especially if you consume a lot of red meat, alcohol and low calcium.

I'm currently experiencing the same, iv been eating more red meat no carbs etc for around a year, TT is within range but SHBG is sky high in turn lowering free T

@@surenotattoos The carnivore MDs don't tell you this. Makes you wonder why? Same happened to Paul Saladino after 5 years of no carbs. Coffee, green tea and black tea all suppress iron absorption. Also calcium from dairy. You are only a year in so fingers crossed you can fix this and get everything balanced out again. Long term, the excess iron can be stored in the organs and could even cause serious health problems. Try cutting out red meat for a while. Eat plenty of eggs, 6 per day for adequate cholesterol to keep your hormone production going. Add some carbs from fruits and vegetables. And also consume poultry and fish for protein. For fat intake use copius amounts of extra virgin olive oil drizzled over vegetables. Try an adaptogenic herb such as ginseng to try and kick start some balance in your body and consume raw ginger, raw garlic and eat chillies and spicy food regularly say 2 to 3x per week. Garlic ginger and chillies are good for T. Also an L carnitine supplement will help stimulate androgen sensitivity and so will chillies. Creatine will increase DHT. Excess iron does not go away easily so try everything you can to optimise your health.

Hi Danny, great video. I am a 57 year old male, 5'7" tall and 143lbs. fairly active, lift weights, cycle, swim, run. Former triathlete. Lately I am feeling more sluggish so did some blood work for a TRT clinic here are my results; Total Test 1180 ng/dl, SHBG 119 nmol/L, Free Test 10 ng/dl And the doctor said I would not be a good candidate for TRT because of my high test levels. Even though my free is only 10 ng/dl and Bioavailable test is 253 ng/dl

I am the same! 5'7", 143 lbs, but I'm 28. I have the same levels and I'm at the end of my patience. I feel so terrible but live such a healthy lifestyle and yet TRT seems to be the only option. Have you found relief?

@@blinkyschannel Like you guys, I have the same issue. Very high total testosterone but also very high SHBG levels and low free testosterone. The male clinic near my house said that I don’t qualify for TRT treatments but I did purchase some proviron when I was in Spain and have taking it for the past few weeks and feeling great (like I am 18 again).

My total testosterone is 1178 ng/dl. Free T is only 1.57 ng/dl. Shbg is 50. I’m on trt. What would you recommend? Single on the higher dose injections?

NO, hes using an example of someone with Total test of 900 and a very high SHBG of 105 which would equate to a Free level around 9, that person would need to raise the dose to overcome the high SHBG to get the free teat up into the optimal range of between 24-40 generally speaking.

@@simonm1135 I’m not a doctor buddy… but many times i’ve seen people who think testosterone to be the cure for all the bad aspect. I mean being tired, fatigue, lack of motivation, maybe a light depression, lack of libido… all of these symptoms are so wide and various that the cause could be litterally everywhere. I pass from 110ng/dL (total T) to plus 3400ng/dL (i was on more than 1gram of test per week) and i can tell you… not so mich of a difference. If the problems in your life are somewhere else… testosterone can do really really close to nothing for you.

@rickycarfan54 Gotcha. My total test is 1190 and free is 6.6 but my shbg is 129 which is outrageous. I have Hashimoto's Thyroiditis as well so I'm just a world of problems. Lol

Question about whether or not I am a legitimate candidate for TRT... I am a 43 yr. old male and have all of the symptoms of low T. Sleep issues, brain fog, moderate level of depression, difficulty losing weight and putting on muscle, and lethargy especially in the afternoons etc. My labwork has been done 6 times or so over the past few years. Total Testosterone levels range from 525-840 in the a.m. and around 412 in the afternoon. My Free testosterone is normally about 6.4 (slightly below the reference range); Estrogen levels are usually around 22-24. SHGB is 51; LH is 8.2; FSH is 6.0 (I have also seen it sky high out of the reference range in hyperdrive.) TSH is 2.09. I am also a type 2 diabetic but with an A1C of 5.2. Unfortunately, my insulin resistance has also been getting worse despite a meticulous diet, so now I take 22 iu of Tresiba basal insulin daily along with 7mg of oral Semaglutide. Would my low T symptoms along with my low free T/High SHGB levels justify starting TRT? Please help me as I can't continue feeling the way I currently do. Thank you for any input.

what does the meticulous diet consist of my dude? It sucks that your insulin resistance is getting worse, it really does, but it means that something about your diet or your meal frequency is off. I'd be more than happy to provide a few thoughts to help you out but I would need more details. You can reverse type 2 diabetes and get back into metabolically healthy catogory, you just need to address the root causes as opposed to the symptoms.

@@jamessanders145 1750 Calories per day consumed in 4 meals. I am 5-8” 165 at 10% bf. Carbs are low glycemic index around 80-180 per day. Protein at .7-.9 grams per lb. When I did keto for 3 years I had no insulin issues but it looks like I may have LADA as my C-Peptide serum and insulin test showed very low insulin production. Tell me, how can you “reverse” diabetes if beta cell apoptosis has already occurred, meaning let’s just say 60% of my beta cells have died? I know GLP-1 agonist have been shown to create new beta cells but so far it hasn’t worked for me. Thanks for any input.

@@fabguy276 Apparently stress can increase cortisol, which messes up blood sugar, even when the diet is perfect. Perhaps it will help matters to reduce stress somehow.

Would someone help me to understand my testosterone numbers? Total: 572 ng/dl SHBG: 41 mmo/L ALBUMIN: 4.3 g/dl FREE: 66.2 pg/ml BIOAVAILABLE: 130.4 ng/dl I am trying to back into how I feel and identify if it possible can have something to do with my testosterone numbers. I'm 63, 6'1" and weigh 210. I've just started on TRT (One dose) Was prescribed 1/2 ml of Testosterone cyponate and an AI twice weekly. Any feedback is greatly appreciated!

Actually your free T is quite low. You have to convert everything to ng/dl, so your total T is 889 ng/dl and your free T is only 6 ng/dl. Probably your SHBG is to high...

From what I've seen it's hard to lower SHBG naturally. Probably something going on in the liver but it can be hard to fish that out, especially when blood tests don't show anything obvious. Of course there are also genes for high SHBG so who knows if you can get around that.

20 years old here and diet is great only thing i can for sure say is i'm lacking on sleep and recovery i train 5-6 days a week and lifting heavy with good form. Just hit the 2 year mark of training naturally and got from 150lbs to 180lb around 11-12% bodyfat (just a guess im pretty lean) I put on muscle and i just did bloodwork with a buddy who is in his 30s who was just like its good to look at ur levels and bloodwork. So i got my total testosterone at 498ng/dl and my free testosterone was only 8.9pg/ml. Not sure what to do here?

Youre a 1.7% free t based on ONE test, you're fine brah. It becomes an issue once you start having low energy, little or no libido, chronic fatigue, etc. Keep doing what you're doing. You'll slow down as you ago, but you're still only 20!

1 shot a week would supress your SHBG more....but wouldn't it RAISE your estradiol more....which raises SHBG? (or do I have that completely wrong, lol?)

Yes, you risk more of the testosterone getting converted to E2 and DHT. Not only that but your RBCs, hematocrit, and hemoglobin could be an issue as well. SHBG binds E2 and Testosterone. If you naturally lower E2 then you will free up more T by lowering SHBG or you should theoretically. So, lowering SHBG by injecting once per week will free up more T, but your E2 will also increase significantly which is NOT good if the ratios get out of whack. A good healthy ratio of E2 to total T is about 4%. Anything you can do to support liver health like using TUDCA, milk thistle, LiverCare, infrared sauna, and getting more fiber in your diet to bind estrogen should help to drive down E2.

Will TRT to reduce my high levels of SHBG reduce my fat distribution from my lower body? I'm tired of these pesky thighs in spite of lots of intermittent fasting, a sensible diet and plenty of time in the gym, including on the treadmill.

My estrogen is high. Sensitive areas on my chest. Had to start taking daily shots and has helped 80 percent on sensitivity. But my shbg is high. What do I do? Take one shot a week to drop the shbg and use an ai to stop the sensitivity? On 110 mg a week and I’m super sensitive to estrogen.

My urologist has me on 100mg Testosterone Cypionate injected into my Deltoid once per week. He does my bloodwork and only tells me what my total T is and never mentions Free T.. What should I ask him.. I see him next week..

Just ask to check your free testosterone. Or ask for a copy of the bloodwork for your own personal records. I asked my doctor and he did it with no real issues. It probably depends on what country/state you're in. I had another doctor who refused, so I just ordered my own bloodwork. It was only like 200$ for a huge panel of bloodwork. In depth Metabolic, FSH and LH, Shbg, Free test, Total T, Estradiol, etc.. I'm on Test Cypionate as well. But, I'm taking 140 mg split into Bi-weekly or Every other day injections. Allows for more stable blood serum levels. Good luck.

As long as he tests total T, SHBG and Albumin... You came calculate your Free T using an online calculador.

@@HF1600ie True, but calculations can be way off. For example, my calculated free T was in the low 114s or 11.4, BUT my DIRECT free T was showing something different at 52 or 5.2. That is about half which is pretty significant. Always measure direct free T just to be sure.

So just to share what happened to me mine dropped to 15 and I started fasting 16-17 hours per day for a few weeks and it rose to 28. first meal at noon heavy on fats and protein. Then normal dinner. Just something to think about. Also Ive heard TRT can pummel shbg. So theres that. So you dont have any shuttling hormone to carry around the goods. That sucks your basically just over filling your body with test that cant be used. Im no dr though.

High SHBG without TRT is diet and lifestyle related. I had high SHBG and once I switched to an ancestral diet, it can down to “healthy” levels. Mostly animal fats and proteins while eliminating seed oils and simple sugars.

Two things raised my SHBG to 124 1. Overtraining 2. Not eating enough fats/carbs Once I corrected both, my SHBG dropped.

These are close to my numbers. 450 total, 8 free, and 40 SHBG. I always feel tired and unmotivated to do much. I thought I had adrenal fatigue or chronic fatigue until I stumbled upon all this info on testosterone. I just had labs drawn last week and have my follow up appt in two days. I think I’m gonna start but I’m also interested in hearing the prescribers input on enclomiphene first.

@@FTJ-4486 I think you are right. My LH was 4.1 which is on the lower end of normal. I think the normal range is 2.5-15ish. I don’t plan to have kids but I don’t want my boys to get any smaller! 😂 Did you make a decision on starting TRT?

Same age as you, total T was 560 but free rest was 6.1! I’m on TRT now, 75mgs once per week.

Holy shit this video is a giant pile of fail. High SHBG = low Free T is not always true, Danny is completely off base here. The first clue is when he mentions that healthier guys tend to have higher SHBG, and that low SHBG is associated with early mortality (which is true). A Equilibrium Dialysis Free T test is needed to get an accurate number. SHBG has a non linear binding affinity for T as it's concentration increases, meaning the more SHBG you have the less tightly it binds to T. There is also evidence that many cells have SHBG receptors through which androgens enter them. Lastly, Boron does not lower SHBG, it lowers E2.
