If you take a look at the scorecard of the Hyederabad Test, you would know that why Imran declared when he did. Miandad at 280* was clearly a secondary issue. It was the day-3 and only the 1st innings was going on. He had to declare to have a chance at winning the match, which we won deep into the final day, thanks to Imran's heroics, as you mentioned, against a very strong Indian batting line-up. Team should come first, not any individual record.

Pakistan white-washed NZ at home in 1990/91, not 1989, under Miandad's captaincy when Imran refused to play against a weak, Hadlee-less, Kiwis. On the 1988/89 tour to NZ however, Miandad did score a big double hundred, but the series, played on placid tracks, ended in a 0-0 draw, Imran was the captain.

Actually the revolt against Javed Miandad in 1993 was orchestrated by Rameez Raja who was vice captain at that time and Wasim was getting vice captaincy at first, but the president of PCB at that time Justice Nasim Hassan Shah (late) made Wasim Akram captain and appointed Waqar the vice captain which I think was the ugly decision. Rameez once again lead the revolt against Wasim after just one year.

Imran may have missed out on selecting some players but he created more stars than any other captain. Imran indeed announced his retirement in advance and the semi-final in Lahore was considered to be his last match in Pakistan. Fans were seen with signs as they implored Imran to take back his decision.
