Honestly, this version must be bugged. There is no other explanation. Every time i tried dragging the body in front of the door, the guard immediately opened fire. How this dude is getting away with it, is beyond me. To everyone who tries to copy this method. Don't. Wait for the patient to explore the offices until he stops in the last one. Then drag the body through the middle of the hall towards the hiding space. Don't try the sides.

The key for the room is in the room at the bottom left of the map clearly marked "STORAGE", in a space in the wall facing north. The room has the red logo on the map; Temple First Floor.

looking through extracted/ripped audio files didn't 47 say "Uhmm... Elsa?" (Probably was changed in a patch, i looked in the steam version one) then Von Kamprad will say: "Vas? You are not the [actual] Lord Sinclair! Guards! Guards!" then i think she would run I think i haven't got past St Petersburg Stakeout sneaky enough or at all to see for myself

I actually managed to fine-tune this method. Snuck in via the sewers and took the hidden chute up. This way I only had to sedate the one guard in the room west of the key and poison. After that I took the cultist robe in the nearby room, got over to the top-east romm w a patient dress and after that followed your tactics for the kill. When getting out, I used the same sewer chute as on the way in. Hence, I didn't have to kill a guard for a costume. :)
