Having done quite a bit of research into Elagabalus at uni I can honestly say that much of the 'history' of his 4 years is probably a combination of myth, lies, repetition and prejudicial writing. Our main source for his short life, the Scriptores Historiae Augustae is a problematic text at best, and the issues relating to Elagabalus's religious policy plus the domination of his reign by his mother Julia Soaemias Bassiana means he fitted into the same model for 'evil despot' first seen with Nero

Elagabalus was one of the most controversial Roman emperors. Not only was his dynasty of Syrian (i.e. non-Italian) origins, but he also replaced the Roman head-god Jupiter with the Syrian god Elagabalus, married four wives and had many concubines, violated a Vestial Virgin, allegedly prostituted himself, had an affair with another man, pulled many pranks, and invented the whoopee cushion... all before he was even 18 (when he was assassinated when Roman citizens eventually had enough).

It's worth noting that many of the outrageous acts attributed to Elagabalus came from the Roman writer Cassius Dio, who'd never seen him but was reporting rumours he heard. Also, Dio suggested that some Romans at the time viewed Syrians, Persians and Parthians as "perverse, superstitious, effeminate", "not real men", and/or "not fit for the throne of Rome". Rome wasn't used to being ruled by a non-Italian (in this case Syrian) foreigner, so that could be a reason for many of the rumours.

Elagabalus was trans, acted like every other spoiled teenager, and had a grandmother who was a kingmaker and manipulated her. Her grandmother got the Roman army to kill her (she was unpopular with the military for being feminine, a foreigner from Syria, and bringing in a new religion to Rome from Syria that revolved around the sun which is where the name Heliogabalus comes from). Apparently she staged a naval battle in a stadium by filling it with wine and nearly suffocated a bunch of guests at a banquet by having tonnes of rose petals dumped on them. Funny stuff

The problem wasn't that he slept with men, as far as the gay sex, is concerned. It's that he played the "feminine"/submissive role while doing it. That was long verboten for any Roman citizen.
