Hi, the gauge of felting needles refers to the width. They're really tiny, so there's a very small difference between the gauges

Would it work on dolls made out of yarn?

I'm not sure, I think it would depend on how tightly the head is knitted/crocheted. I think the best way to find out would be to make a simple doll head and test it. If you can't felt directly into the head, you could perhaps make a needle felted wig using a piece of felt, then stitch that to the doll head?
