How Do Ozempic and Wegovy Work?
Ozempic and Wegovy are synthetic GLP-1 molecules named Semaglutide. Ozempic is approved for type II diabetes mellitus and protection against cardiac events in type II DM patients. Wegovy is approved for weight loss in obese and overweight individuals and for protection against cardiac events in obese and overweight individuals. Let's review the mechanism of action and the side effects of these drugs.

Haven't we learned yet that taking a pill to lose weight never turns out well. I have gone from 235 to 175 in about 9 months by not eating highly/ultra processed foods, limiting added sugar to no more than 25 grams a day though I usually do 2-6 on average a day. I switched to healthy breads one is Rustik sourdough and has no added sugar and used extra virgin olive oil and Ezekiel's 4-9 bread, all sprouts no flour or oil and is fantastic! Just switched to avocado based salad dressing too. I walk lightly, I have stay stay low impact so I know the food is the bigger factor!

Never turns out well? My buddy has lost 60 lbs and stopped alcohol on Zepbound so far.. turning out pretty fine from what I see

Glad to hear that worked for you but it's not that easy and straight forward for everyone. These class of drugs help with people who struggle with food noise and bingeing which are the mental component of weight issues. Prior to semaglutide, I was on a yoyo variety of diets for 30 years with zero successes only frustration and misery. This has been a life saver on so many levels especially mentally.

Dr Been, The glp-1RA lasts for ~hours~ as opposed to natural glp-1 that only lasts for ~minutes~. This is why hyperinsulinemia and hypoglycemia occur with this drug. It will start out not being as obvious in a highly insulin resistant individual but if an individual has little or no insulin resistance they will get hyperinsulinemia and hypoglycemia. The hyperinsulinemia will then ~cause~ insulin resistance! Do 6 hour oral glucose tolerance test plus insulin and c-peptide to actually see what this drug does before, during and after treatment with this drug.

No long term studies...yet fda approved. We need to change the way fda gets paid by pharma (80 percent). How is that NOT a conflict of interest? There are more drug commercials on tv than processed food now. But lets advertize all that crap so it keeps people buying their pathetic drugs. Drugs are the answer....give me a break!

What about the Eli Lilly version ? Mounjaro/Zepbound..the active ingredient in that case is tirzepatide .. does it work exactly the same as semaglutide ?

UC Berkeley found that the effectors of glp-1/glp-1R is TRPA1 and TRPV1 on the sensory neurons of the lymphatic system. This is the neurolymphocrine system. The signal travels from the lymphatic system to peripheral and central nervous systems where it stimulates insulin secretion and satiety it also inhibits GnRH inhibition -> excess LH and FSH from the pituitary. FSH is the main stimulant of SHBG production that binds estrogen inactivating estrogen. This leads to not enough estrogen control of osteoclast maturation and this leads to bone loss. A preprint study in The Lancet shows a double blind study where semaglutide leads to bone loss in susceptible adults. Glp-1/TRPV1 in the peripheral neuron stimulates sodium excretion in the kidney and this leads to hyponatremia/hypoosmolality and bone break down.

Thanks for the great discussion on GLP-1 and this drug. A Rare Sugar call Allulose, which is very similar to Fructose but also works thru the GLP-1 pathway in a more natural way. I found the discussion of the half-life going from minutes GLP-1 to a week for Semaglutide interesting. Allulose stimulating GLP-1 would keep the control system more intact when added at meal time, instead of completely obliterating the Control System for weeks at a time.

yea my friend (an avid liquor drinker) stopped alcohol cold turkey on Zepbound.. he says "He lost the taste for it"

Yep. Seeing lots of addictive behaviors stopped. I'm thinking it may have more to do with the GIP component.

*From Google* In total, the FDA's system has recorded 117 fatalities among people taking blockbuster weight loss drugs since 2018. Of these, 81 were linked to patients using semaglutide - the active ingredient in Ozempic.Serious side effects, including allergic reactions, hypoglycemia (low blood sugar), increased thyroid cancer risk, gallstones, and swelling of the pancreas. Regaining weight when no longer taking the medication if lifestyle changes haven't been made. *I think it is a dangerous untested option*

Finally watching/listening to this lecture. Hearing that hypothalamus knowing the insulin level and thus knows glucose is present. Maybe our glucose testing should learn from hypothalamus and measure insulin too.

I am not a dr but I do have a FB page on Ozempic side effects. I shared this video there for the others. I would be interested in asking you some questions. I was on Ozempic for 5 months and it dropped my A1C from 12.7 to 6.8 in that short time. It also caused inflammation in my eyes which caused me to have optic neuropathy in both eyes so I am legally blind now. The makers of this drug acknowledge retinopathy and blurry vision but not optic neuropathy. I often wonder if it was because my blood sugars plummeted so low. I would love to hear your feedback. The next question is why a large percent of the people who stop taking Ozempic get extremely hard to control blood sugars. I have been on a strict carnivore diet and my blood sugars still are around 150 and if I cheat they skyrocket even up into the 500's. Why is this happening it is as if all my liver wants to do is crank out glucagon and keep my sugar high. My Drs don't listen they are too busy protecting Ozempic so I am left to try and figure this out on my own. That is what led me to your video. And as a side note I would have kept listening even if it were longer ☺️ thank you for posting it.

Thank you for a great lecture. Have to watch it again, to better understand. I know about Ozempic, but in my case there were things you explained that I found useful for Gastroparesis. My daughter got diabetes type 1, and know suffers severe G. Even if she's not fat, maybe Ozempic , or anything related, could ease it.. Only a hasty thought. Gastroparesis is most common in diabetics, but seems like its an increasing problem also among others..

Does the drug reduce serotonin in the gut-so this is why the the action of the colon is slowed.

Metformin does the same thing. It's especially in women with PCOS and it's related infertility. If you can help the insulin resistance, you can help the hormones issues at play...thus helping regulate cycles and ovulation and increase chance of pregnancy. There is also the caution of taking oral contraceptives with the slowed gastric emptying that might interfere with absorption.

@@nickyrivernene5921 I tried vegetarian for like 9 months. Ended it after episodes of narcolepsy. Then was a healthy clean eater for 10 years. Became pre diabetic, hypertension stage two, near clinical tachycardia, autoimmune issues, near IBS. Keto fixed everything and tons of other issues. Not to mention energy through the roof, no more old man gut fat, looking hotter than I did in my 20s. Now getting down plant eating to zero. No way am I going back 😉
