the media is misleading. THe two vaccine gives different types of immunity. THe problem with Western vaccine , is their defense maybe stronger, but it is monolithic, and will go obsolete. all of them have the flaw of only attacking 4 antigens on the spike. Chinese attack the entire virus. It is definitely not as good in preventing illness. THe Western vaccine is geared to give a strong and quick response, but the Chinese will continue to prevent serious illness when mutation makes the other vaccine useless. Also, since inactivated viruses will be imprinted in the bone marrow, need for boosters should not be there as antibodies will be produced whenever required

Simply put, incidence of a transmissible disease is the result of a complex and wide set of variables. These factors include existing (and cross) immunity, factors related to the pathogen itself (in this case, variants of SARS-CoV-2), individual behaviors (mask usage, travel habits, etc), and societal factors. Vaccines also play a role in that. On top of that are large differences between countries in how infections are detected and reported. Altogether, observations in incidence across countries don't provide any information about any single factor. Instead, as I shared in this video, factors have to be isolated and confounding factors have to be accounted for in order to understand the role of individual factors. This is basic idea behind study design and is the core curriculum in a public health or epidemiology program. On the other hand, if you make an observation (e.g., hey, I see that X country used Y vaccine and they had an unusual trend in Covid incidence--good or bad--I wonder why that is?), you can then design a study that can try to answer that question systematically. This is what public health researchers do and it can be done retroactively, given the right set of data with sufficient sample size. So, you ask a good question: why do I see what I see? With sufficient specificity in your question and the right data, the question can usually be answered.

Just to clarify something: Chile did not “use only Sinovac.” Around 80-85% of the vaccination doses administered in Chile were Sinovac. (see supplementary appendix of NEJM study) Also, this is really important, so if nothing else, take away this lesson about epidemiology, study design, and confounding factors. Vaccinations are not the only factor determining Covid incidence in a country. Comparing 2 countries at a specific point in time (eg choosing today) does not tell you anything. If the number of susceptible people is low (because they’ve been infected from a previous wave), if mask usage is high, etc-these factors are extremely important. Choosing a specific point in time also neglects what happened in the past. You can find any country is “lower” than any country, if you choose the right time. You’re not actually measuring the factor that is at play. This is just introductory epidemiology and study design concepts. A simple example I typically give in an introductory course is: in a town, ice cream sales and violent crime are highly correlated, being high in summer and low in winter. Does ice cream cause people to commit violent crime?

Thank you. I got both doses of SINOVAC in April and in May . I have comorbids. Early August I got infected but symptoms were very mild. Just took a Paracetamol for the 3 days of 38 degree fever and full dosages of ESBERITOX N (immune system booster). My whole household got infected. All fully vaccinated and everyone got it mild. The first 12 months I was not vaccinated. I have compromised immunity and have high risk of dying if I contract the coronavirus. So don't let your guard off and you should get yourself vaccinated for protection.

I was approved to work at home for the last 18 months until this September. I try to keep distance as much as I can. I shop weekly on a busy street, sometimes shoulder to shoulder, but I leave the place as soon as I can and shop early when there are less people. I had eaten in restaurants a few times during the last 2 months. The restaurants still keep tables with at least 3 feet distance and only people fully vaccinated can enter. Since last month, September, I need to ride in crowded subway commute between work and home. I am a little worry for there are still people carrying the virus without knowing. But it is good to go back to work to see other people. So far I am ok so I believe I didn't expose to this virus directly yet. I just looked up the statistic. My living neighborhood has 99% adults fully vaccinated, my work neighborhood 90% adults fully vaccinated, my work place 100% adults fully vaccinated but with 300+ kids younger than 12 not vaccinated. I am glad I don't need to directly work with the students any more. There is random testing weekly for the children and no sick child is allowed into the school, temperature screening for all students every morning. The area I need to commute across every morning has only 60% adults fully vaccinated. My greatest risk is when I am on subway and buses during my commute.

There is a problem with this argument, which is that the phase 3 trial in Turkey for Sinovac used healthcare workers, yes. However, that phase 3 trial showed the highest reported effectiveness for Sinovac among any study about Sinovac. In other words, it was the best results. The study from Chile, which used a much bigger sample size and was extremely representative of the overall population (used a cohort that was 80% of total population), found a lower effectiveness. Therefore, occupation of the sample is not the only or most important factor affecting effectiveness. I think what you’re implying is that healthcare workers are at higher risk to exposure than the general population. And, you are right (although they also use PPE and are more usually more risk averse in their actions). However, effectiveness in a phase 3 trial (or any properly designed study) compares equivalent populations of vaccinated and unvaccinated. So the vaccinated doctors may be at higher risk compared to general population but they are being compared to unvaccinated doctors that are doing the same things and have the same level of risk. This is why this factor is not substantial. Instead, what is more influential on a vaccine’s reported effectiveness are the factors discussed in this video, such as variants. Keep in mind the study you’re talking about in turkey used a very small sample size, which resulted in a high level of uncertainty (95% CI ranged anywhere from about 60-90%), meaning the real effectiveness in that HCW population likely was anywhere in that range. The Chile study was not a phase 3 trial but was a much stronger study design and had much larger sample, resulting in very low uncertainty (95% CI was only +/- ~1-2%). Hope this helps you understand how effectiveness is calculated and the importance of accounting for factors in study design.

I always see the same mistake when reading the results of the Chile study, and I can say it with confidence because I saw a video with the director of the study that explained the results. It´s 65.9% of infection, but of the rest is where you get the other results and so on. So from 34,1 is where you get the 87,5% hospitalization rate and so on. He said that Sinovac with they saw a 99.9% effective against death. And you can see that in the results. 4´173.524 had two dosis. From the 10 million, died 4042. If we only take the two vaccinated and the deaths, those deaths are the 0,09%. If we took all the 10 million in the study, that percentage would be even lower (0,03%). So Sinovac is like 97% against hospitalization, and 99.9% against death. Check you own data and you will see it.

Yes, the outcome against which effectiveness is measured must be specified, as I do in this video. Just so we don’t get confused here is the text from the study verbatim (link in description): “ Among persons who were fully immunized, the adjusted vaccine effectiveness was 65.9% (95% confidence interval [CI], 65.2 to 66.6) for the prevention of Covid-19 and 87.5% (95% CI, 86.7 to 88.2) for the prevention of hospitalization, 90.3% (95% CI, 89.1 to 91.4) for the prevention of ICU admission, and 86.3% (95% CI, 84.5 to 87.9) for the prevention of Covid-19-related death.” The lower effectiveness number you report is, as stated, against symptomatic infection. Vaccines are helpful in reducing the likelihood of hospitalization and death, even if breakthrough infection occurs. The key is to compare apples to apples (equivalent outcome measured in equivalent ways). Not sure where you get 99% from. It sounds like you’re just taking total deaths divided by total cohort size, but that’s not how it’s calculated according to study design and statistics. You can read full report to see methods. I can also suggest good introductory courses on epidemiology and study design on Coursera (free) if you’re interested in learning more.

I would asume that if you get into ICU, the prevention of death should be higher because you first get into ICU and then you die, so the number of people who get into ICU should be higher than the death toll, no? the numbers say otherwise, they say the death risk is higher than the ICU risk, that´s what I´m reading and it makes no sense.

In Thailand, govt give SinoVac to doctors / health care workers, my gf and some of my friends are one of them. We already knew ahead that the protection could be significantly lower than other vaccine as at that time there is very very limited number of study on the vaccine. So many of my friends and also my gf took the anti body test. The antibody level 1 month after fully vaccinated is relatively low. For 2-3 months after fully vaccinated, some of my friend got result as no antibody at all / negative antibody. I know that there is more than antibody (B-Cell), there is T-Cell which we couldn't measure but I think it is quite a good indication about protection level overall of the vaccine. Sure Sinovac still give some protection but the govt should treat it as interim solution and give those people who received SinoVac a proper booster dose.

you have it backwards. Western Vaccine should be used as an interim. The problem is that it's only made to attack 4 antigens and will go obsolete. Biontech has admitted this already, the maker of Pfizer's vaccine. CHinese might not give strong immunity, but the antigens are stored and will produce antibodies faster than never being infected or vaccinated. the thing is CHinese vaccine is against the whole virus and the virus will not mutate everything away. We know the virus will be here to stay now Who has stated it, so the point of the vaccine is to give us a head start in fighting the disease so it won't be so deadly, oppose to not getting infected at all.

In the Philippines, majority of vaccines shipped to us are Sinovac, although we have used all 6 brands at varying quantities. Since it was first one available, our HCWs were mostly vaccinated with Sinovac; the other option for them was AZ. So far, we have been hearing of breakthrough cases among them. There have been deaths also but usually accompanied by co-morbidities. I chose Sino this April-May bec of the known at that time adverse reactions to AZ. Of course, concerned about waning efficacy, but we're not giving boosters yet. National elections next year, focus is getting more people their 1st and 2nd doses. Protest and educate your leaders. At this point there is no reason to get pfizer. THe Western vaccines will become obsolete eventually. Biontech, who makes it, already said that Pfizer's vaccine will be obsolete in 2022. Chinese vaccine will give some immunity no matter what variants show up, because it's using the whole virus. There is no way the virus will mutate everything. We know the virus won't go away, and the point is not to prevent all infections anymore, so a strong vaccine is not necessary. It's good for protecting the vulnerable, but we either let people get infected now, or use a whole virus vaccine .

You are correct that the population matters. In another video where i do a meta-review of all phase 3 trials, you can see more data on the populations of each vaccine’s phase 3 trial. However, none of them use frontline workers exclusively. The goal of a phase 3 trial is be representative of the overall adult population (age, sex, demographics, etc). The best example of a representative study is out of Chile. That one was not a phase 3 trial. Instead it was a prospective cohort study. The cohort was 80% of the entire country’s population, which is an exceptionally good dataset. One study out of Brazil discussed in this video (also not a phase 3 trial) reported results from a population of healthcare workers (as noted in the video). However, the effectiveness is determined (in any properly designed study) by comparing equivalent people within a given population (that is, comparing vaccinated to unvaccinated). In a population like HCWs, there may be factors that increase overall risk (eg increased risk of exposure) but also factors that decrease risk (eg higher risk prevention measures like wearing PPE and higher knowledge about protective measures), so these counteracting factors would create variable differences in that population compared to a non-HCW population. Nevertheless, effectiveness is not about overall risk but relative risk compared to unvaccinated within an equivalent population. It is not about relative risk between different populations. Like I pointed out in the video, overall trends in a population cannot tell you any information about any vaccine. The reason is because you need to isolate the factor you’re interested in. You have to know who has the vaccine and who doesn’t and who got sick and didn’t get sick. Many factors (behaviors: mask usage, regulations on in person dining; environment: weather, time indoors; introduction of immune evasive variants, etc) affect epidemiological trends. If you want to know what role a vaccine played in that, you need a study design that can answer that question. Looking at single points in time and a single population is poor science. In much the same way, we can see that countries like Seychelles, Mongolia, and many others had extreme outbreaks despite high vaccination rates. In other words, regardless of the vaccine (Pfizer, Sinovac, sinopharm which is biologically almost identical to Sinovac) this has happened. Because not everyone is vaccinated (look at the US) and because no vaccine is 100% effective at preventing infection. But these single point observations don’t provide any meaningful comparisons across countries. If you wanted to use this kind of data, you could do an “ecological” study design, in which you looked at incidence rates in every country over a sufficiently long period of time (4-6 months) and looked at vaccination rates and vaccine types in each country. The aggregate of each a country would create one single data point. With 200+ countries, you might start to see some weak correlations between incidence and vaccine factors (eg brand). But with so many other factors confounding it, it’ll be hard to tease out. My goal is not to convince people that vaccine A is “better” then vaccine B. I’ve taught study design for many years so my goal is to help people understand how (and how not) conclusions can be drawn from data. Hope this helps. Because Chile and Brazil have suffered enormously already. They have reached this low incidence today through natural infection. Considering only reported cases (probably only 20-50% of actual cases).. Brazil has reported that >10% of its population as having had Covid.. 8.5% for Chile.. while Singapore is 2.5%. So 300 to 500% higher in Brazil and Chile compared to Singapore. To be clear, though, I don’t attribute this difference to vaccines. Policy and behaviors play a substantial role in this observation. We cannot make conclusions on anything based on singular points in time or in trends influenced by many factors. Instead, science requires us to isolate a variable and measure an outcome in order to determine the association. Hence the importance of study design an a thorough understanding of epidemiology and statistics. Great courses on these topics are available for free on Coursera!

Thailand’s policy (where I live) is that anyone that received Sinovac should have a booster as soon as humanly possible.. they are using Pfizer mostly here, but moderna would be equally good. Data shows that following this booster neutralizing antibody levels are high and provide protective levels (meaning something like 90-95% efficacy in preventing symptomatic infection) likely for at least 6 months. I believe the booster is no different in dose from regular 2-dose regimen. That’s what Thailand is doing. I’m not aware of any study using a sub-dosing for boosting. My recommendation for people is to measure neutralizing antibody levels and if they are low (eg

Antibodies are all about tagging the virus from spreading and cloning inside the cell and so on. And during school years, we were taught the antibodies have 4 types of mechanism, one of it is the Neutralization. All of the mechanism later on as tagging which will the phagocyte by the macrophages and also other phagocytic cells. If there is no tagging for the virus, other than antibodies such as t-cells can also be triggered for the immune response.

Most data suggests that a booster should be something other than Sinovac. Most countries are using Astra or Pfizer to provide a 3rd booster to those previously inoculated with Sinovac. The levels of neutralizing antibodies from Sinovac are generally high for only about 1-2 months. After that time, boosting is needed.

I think all vaccine at this current point in time have reduced effectiveness in preventing infection. What is now most important is the effectiveness in reducing hospitalisation, reduce oxygen supplementation, ICU and death. Are there any real-world, large studies on these?

Absolutely. All studies shared here are real world. They are regular people living regular lives. The studies out of Chile are some of the best because they have a national social health insurance program (so very large and comprehensive dataset). You can also check out my channel, where I have a video on the Delta variant which includes several more “real world” studies. (In fact, I work in public health, not microbiology or virology, so everything about epidemiology is real world and not lab-based.) Overall, many countries built public policy on the expectation that vaccines would create so-called herd immunity and stop transmission; however, while that has not materialized, the silver lining is that vaccines reduce severe outcomes (death etc) against current variants for a finite period of time (probably 6 months to a few years, depending on which vaccine).

So verocell is actually referring to the lineage of cell cultures.. they have been used to (among other things) produce the SARS-CoV-2 virions that are used to make both the Sinopharm and Sinovac inactivated virus Covid vaccines.. therefore, if you see “verocell” on your vaccine box, it may be either sinopharm or Sinovac. Check for the brand name. The data shows that one dose of Sinovac is essentially 0% effective. Several of the studies shown here (links in video description) report on effectiveness and/or also neutralizing antibody levels in people that received only one dose. One dose also would not be considered “fully vaccinated” (depending on where you live and where you want to travel, this may be important). Therefore, you may want to consider finishing the inoculation. Then, depending on your risk, you will need to consider a booster in 4-6 months following that (this is what data collected and reported by Sinovac says.. that after 6 months basically no measurable neutralizing antibodies).

Yeah, many of the studies discussed in this video are from Chile. They have an excellent dataset since they have a national social health insurance program (that means big data set).

Hello thankl you for the information on the Sinovac vaccine. I have been triple vaccinated so my booster was Sinovac. I find, unfortunately that the Countries I need to visit do not accept Sinovac. It is very sad that a fully vaccinated person is treated as if they have not bothered to be vaccinated at all. I hope this will change.

What about Sinopharm BIBP (use in Asia, Africa, South America, and Europe) and Covaxin - Bharat Biotech (India)? How do they compare? I think adverse reaction comparison would be also very interesting. Not yet released for emergency use are Novavax (Spikeprotein vaccine) and VLA2001 (Valneva/Dynavax inactivated virus).

Sinopharm is nearly exactly identical to Sinovac. The only difference is a slightly different starting virus in the manufacturing process. (The mutational difference is insignificant). As for Bharat, I’m not familiar with their process enough to comment.

@@AnthonyKuster Thank you! I would like to understand more how much the quantity of mRNA/Vector, Spike-Proteins or inactivated Virus influence the effectiveness and toxicity (adverse reactions). Moderna despite having the highest Antibody persistence seems to become banned for younger people (especially younger men) in a lot of countries, so that only Pfizer remains since also AZ and J&J are not allowed for the younger (women).

Sinopharm is biologically and chemically almost identical to Sinovac. Created same way, same mechanisms. Therefore, true effectiveness would not be significantly different.

I got Covid last year and this year I took Sinovac. I felt nothing, no sore arm or anything. Maybe my immune system was still able to just shrug off the virus without any fuss.
