I would have set Up to move Forward, Down to move Backward, Left and Right just to turn. I know there's no strafing but you can move and look around, at least. Start should be for the Pip Boy menu. Select should be to reload/draw. B as Fire A as Interact New Vegas has a few distinct control issues, mainly to do with VATS. Without the ability to look up or down, you would have a very hard time in firefights without VATS. I theorize that whatever button registers your VATS input should be set to A, and also a button to cancel VATS should be set to select. B will, after all, be the button to fire with.

When I say "register your VATS input" I mean this; VATS: You initiate VATS; The target shows. You input Fire after selecting body parts You hit the button to end VATS and begin shooting. The last button you hit before VATS makes you start firing should be set to a button other than B or A, merely to differenciate between which buttons select, fire, and cancel.
