How pacemakers work. Animated explanation of the mechanics of the human heart, and the devices that can assist it

Implantable pacemakers and defibrillators are devices that apply electric shocks to maintain the rhythm of the heart and, if necessary, restart it.
As the technology improves and the list of treatable conditions grows, the number of devices being implanted is increasing steadily and now exceeds half a million a year.
The heart is made up of four chambers; two atria and two ventricles. On each side the atrium is connected to the ventricle by a one-way valve. Blood is pumped as these chambers contract and relax in turn.
The beating of a healthy heart is regulated by electrical impulses. The sequence begins as the atria fill with deoxygenated blood from the body on the right and oxygenated blood from the lungs on the left. An electrical signal from the sinoatrial node then causes the atria to contract, forcing blood into the ventricles. The electrical signal is then picked up by the atrioventricular node and directed into the Purkinje fibers in the ventricle walls, causing the ventricles to contract, and the blood is then pumped through the pulmonary valve on the right to the lungs, and the aortic valve on the left to the rest of the body. These valves close and the cycle then restarts.
When the sinoatrial node fails to function correctly an artificial pacemaker can be fitted to help regulate the heartbeat with small evenly timed electric shocks. This involves implanting electrodes into one or more of the heart's chambers, by inserting leads into a vein near the collarbone and implanting a device called the generator just under the skin.
For more severe heart conditions an implantable defibrillator or ICD can be used which is also capable of sensing a stopped heart and delivering an electric shock powerful enough to restart it.
For some conditions an even more sophisticated device called a CRT ICD can be implanted. This uses a third lead inserted into the left ventricle to resynchronize the ventricles when necessary. However, all these leads can cause problems of their own. Patients with ICDs have a 20% chance of a lead failure within 10 years and replacing leads can require open-heart surgery in about 2% of cases.
This has resulted in several efforts to develop new pacemakers that do not depend on leads inside the heart. One design, the subcutaneous ICD, places the lead just outside the heart under the patient's skin.
And wireless designs are now being developed that may eventually do away with the need for leads altogether.

From the book … The Spark of Life: Electricity in the Human Body … author … Frances Ashcroft Early pacemakers were large and bulky machines, about the size of a washing machine, and they were supplied with mains electricity. Consequently, the patient could not move around easily. They also had another disadvantage: they stopped when the electricity supply failed.

Boston Scientific Dual Chamber Pacemaker. Got one now. Works Great so far. Only about the size of 3 stacked Quarters. I just have to remember that I have it at times. Like when I'm lifting, or working on my car. Or walking through a Metal Detector.

I have a dumb question, what happens when it runs out of battery? Will they have to performa surgery every single time they have the need to replace it? Or is there a sort of extension outside the body? Or are there any alternatives instead of a battery?

Batteries have a lifecycle of at least 10 years, so it's not like you will need a replacement every year.

Heart failure is the reason why you may get ICD if heart beat around 40 beats are slow beating up to the top of the heart valve.

My father had a pacemaker plant 1 year ago, his heart was falling so he had to put it, he does dialysis and it didn't help a lot because dialysis weakens his heart, the only solution is a heart transplant which we are saving up for. We live in Lebanon and it costs 20000$ but with insurance it is 3000$ which what he paid for.

But I don't know really how does it work, she has this problem from childhood but unfortunately we understand it now, but it is not too late, we are living in 2018, very thing is possible, and I will do my best for her to make her healthy and happy. Hi, my doctor said that I need to use a pacemaker since my heart rate is too slow and irregular. But I'm still too afraid to use it since I'm a sport typical person. Is there any side effects from using it like for example you can't move your left hand freely or you can't do physical sports like basketball,dance,etc? thanks for helping :))

@@stephanieelawitachya3529 hello, thought I would try to help answer your question. Although I don't personally have a pacemaker, I do work for a cardiologist as a vascular technologist and quite a bit of my patients have pacemakers implanted. All the patients do just fine with the pacemaker and don't recall any of them complaining about it restricting them in any way. Once the incision heals you will most likely not even know its there and it shouldn't interfere with your mobility or range of motion with that arm at all. Hope this helps some.

@@stephanieelawitachya3529 contact sports and exercises that recruit muscles near where the pacemaker is implanted is not recommend, since it can move the pacemaker or the wires. Jogging, swimming, cycling, or other rhythmic exercises are safe though.

my device had 9/10 expected adverse outcome. and wireless was date . time . location specific. we called it our NO vista upgrade. 2006 shocked 9 times not from rhythm battery dead. orphaned lead. cohort to next boxwood device. 2011 revision 2012 to rib cage. allowing for stress positions and backpacking. 3g wifi. 905am wake. near field communication MHz for stingray . and simply copy exact MHz of a cellphone and repeat. equals delay ..
