Pizza tower is the kind of game that is like buying a pizza from a local pizzeria, you can order the whole pizza and only eat a few slices, but if you want the full experience you gotta sit there and eat the whole pizza. At some points you may feel sick and may even throw up, but afterwards, you feel accomplished. And then once you finish it you never want to touch pizza again.

So one of the characters in the game is named John Pillar, or Pillar John. It's sorta like a pun on Papa John. You gotta beat him up to complete every level, and it starts the whole escape countdown thingy. So John Gutter is a pun on John Pillar, because the first level is a graveyard of decrepit John Pillar thingies. Because it's the abandoned gutter area. It's...not a very cool name or pun. It's like they didn't even try with this level. But you'll probably play it a dozen times because it's so satisfying to play when you are still getting used to the game's mechanics.

It’s actually kind of part of the lore. Apparently Pizzaface was cloning John and using the copies as the support pillars for the tower, and John Gutter is where all the failed ones were sent. You destroy the original at the start of TCTOP.
