Time, HP, Parries and Super Meter all count towards final grade. You need 3 parries, 3 hp and 6 super meter for a+ rank (time changes per level but you want that to turn gold as well ideally - i don't think you need gold for every a+ grade).

That caffeine power up is great! Watching these after i have struggled to a+ myself and i have been going with the additional heart. Not as efficient but i did it so can't complain. My reaction was similar to yours! This game is tough (and fair) but very rewarding when you finally nail it.
