How to Add an Electronic Signature in Google Forms

I spent a long time searching for a platform that would let me create a form with a signature feature, but I was having a lot of trouble finding one that worked well. Why is there no same app (signature) to my workspace😢. Is there any other apps?

when I clicked on multiple signatures, and clicked confirm, it tells me login is required for multiple signatures
