How to assemble the uplight - Home sweet Home Lighting

I have the same model floor lamp but in bronze I've had it for 21yrs. But mine was already fully assembled, but now it keeps leaning forward not standing up straight I will have a look under the base to see if the screws/bolts are fully tightened.

Which bulbs does this take? I have this but no bulbs please let me know where to purchase
