Is it better to start fellowship as soon as residency ends or one can work for an year or two as Internal Medicine attending before jumping into fellowship? Would that decrease the chances of matching in Cardiology?

as an IMG, what is the best cardiology felloship in usa? because i am hearing that cleveland clinic and myo clinic rochester come first and second respectively is that true?, and what is the best invasive cardiology felloship in usa ?, and how to increase my chance in getting into invasive cardiology felloship? Because i am a medical student and i publish meta analysis in plos one journal (but not related to cardiology),and i learn using SPSS and R program for analysis + i participated in Network meta-analysis+ i wrote review articles(but not published yet), my question is : does all my previous experience increase my chance in getting into high ranked cardiology program?, and if i am doing meta analysis +review article is it a valiable experience and give me advantages compared with other? is it important do rank residency internal medicine program according to the best cardiology felloship at the place ?

I've heard that residents works 80 hour weeks and are on call! How does one have free time to do research? Or do you have designated research time while at the hospital performing resident duties?

Sorry for the late reply. The 80 hour work week limit for resident physicians is an average over 4 months. So you can technically work 100 hours week 1 as long as you work 60 hours week 2 to average it out. It can be intense during time intense rotations like the ICU but thankfully not every month is like that. For research, some residency and fellowship programs do offer dedicated research blocks. Other people take a research year. But most commonly residents and fellows do research concurrently during their usual day to day rotations. You basically have to find time! It’s difficult but not impossible.

@@foxbrendenn That's misleading. In the United States, residents and fellows are both paid positions. However they are not paid at the same level as an attending physician. You are paid commensurate with your post-graduate year. So a first year internal medicine resident is a 'PGY-1'. A first year cardiology fellow who has completed the required three year internal medicine residency is a 'PGY-4'. So they are both paid, and in fellowship you are paid more than in residency, but by no means are you paid well enough in either one to 'not count' the years of training required to become an invasive cardiologist.

In order to practice in the US you must redo your residency and fellowship training at an accredited US program

@@MarcKatzMD I know about that but I'm just asking about the cardiology fellowship first like then I can use the reverse pathway to internal medicine after fellowship, or it is not possible in case of cardiology fellowship bcz I think incase of nephrology and infectious disease one can land into fellowship directly....

What do you recommend for student that has never ever done research, not even undergrad level research? Seems like you have had research experience before residency. And do ppl take a research position for a year to boost their application after completing internal medicine residency if they are interested in competitive fellowships? Or most of the research is done during residency? Do you have to publications or unpublished research experience stills looks good on an application? Sorry for the barrage of questions. I’m still a medical student so I have some time to decide on what specialty to pursue but wanna keep my options open in case I want to go for a competitive specialty. Please do a video on just research like all that it entails. How to get involved? Etc. it would be nice video series if you could make a video on each important step you took to get to where you are today starting from medical school.

Sorry for the late reply. All of your questions would make an excellent future video! I did a minimal amount of research in med school and didn't do a robust amount in residency compared to some of my impressive peers. Many people take research years during medical school and even during residency depending on the specialty they are pursuing. But don't worry about lack of experience. Everyone has to start somewhere. During rotations in med school try to talk to residents, fellows, and in particular your attending physicians. They are your best resource to get on a project. Structural heart disease fellowship is typically done after gen cards. So application is during second year of fellowship and often done as an interventional cardiologist

Hey bud- any interventional program would be a great opportunity. It’s hard to rank them from top to bottom but I’m sure there’s got to be a website out there that does so. I don’t know this answer of the top of my head. The first step is getting an IM residency, then general cardiology fellowship, and then interventional. Research in residency > research in medical school. But any research helps. Certainly most important aspect is to get strong training in internal medicine first and foremost

Getting involved in research is always a good idea. As a medical student it can help improve your residency application. For fellowship most programs want to see what you’ve done during residency. So for med students if you can get involved then go for it! I waited until my third year because I focused on acing step 1 back when it wasn’t pass/fail. It was also easier for me to find attending physicians who could guide me in finding publishable research projects that I could have an active part in- things like manual data review and entry. So it depends on who you are as a student. I couldn’t handle research on top of step studying as an MS1 or MS2 but if you think you can handle both then go for it. Talk to your professors. They’re great resources who might be able to help you find a good project for you to get involved in

Hi nice video. I have low board scores but matched into an IM program through hard work. I have close to 40 publications and more than 60 abstracts in well established cardiology journals. My question is does this overcome the low scores and do I still have a chance for the fellowship?

I’m not a program director so obviously take my opinion with a grain of salt. But I feel like that much research should get your foot in the door. Unfortunately PDs still look at board scores as a predictor of you being able to pass specialty boards and IM boards. But high quality research shows your commitment to cardiology IMO

Heart disease is most common in older adults. So the majority of my patients are 40-75 years old. I see far more 85 year olds than 18 year old patients. It’s just the nature of most cardiac diseases. If you want to solely see pediatric patients you can do pediatric residency then pediatric/congenital heart disease patients. But that is also a very unique patient population. You can also see adult congenital heart disease (CHD) patients after a general cardiology fellowship but those are adult patients who have CHD and are certainly younger that other cardiology patients but it depends on what you want to do for the next 30-40 years of your career

No. Pay is terrible as a resident/fellow across the board. It’s just another indicator that you should listen to your gut if you get a bad feeling about a program. No pun intended

I'm not entirely sure of the process as surgical residencies are different but instead of doing an internal medicine residency you would first do some sort of surgical residency. So it would be completely different process that typically involves a 7-8 year surgical residency/fellowship. At the end of the day you still have to do well in medical school

Why didn't you just become a dermatologist? You opted for a non-surgical specialty that made you work way more a day (I assume you work 60+ hrs a week?) for about the same a 8-5 4 days a week derm makes 4 yrs out of med school -- not 6. Did you consider things like anesthesiology, radiology, and derm? If you did, why decide against it? It must've been hard paying off med school while being 3 yr resident, 3 yr fellow, and potentially 1 yr invasive. Hope you can answer. Thanks.

You hit the nail on the head when it comes to the time investment. You’re absolutely right that the ROAD specialties (radiology, optho, anasthesia, derm) are relatively shorter with better quality of life and pretty great pay. The thing is that I just didn’t enjoy those specialties and couldn’t see myself doing that for 30 years. At the end of the day you have to be happy with your career. Medicine can certainly feel like a job sometimes but I simply enjoy cardiology. It’s tough investing 6 years to it but I’ll be doing it for (hopefully) the next 30-40 years of my life. So I’d rather invest now and be happy with a long-term career
