I'm working for an open world game, the map design took me 2 years, don't trust procedurally generated terrain tools because it will look bad and not realistic at all.

Of course, some tech artists use procedural generation and they make it work but it's still months or years of work

i think you mean four concepts? The terrain, the procedurally generated/scattered foliage and rocks etc., texturing and finally .... everything else, lighting, clouds, other post processing stuff and vfx, actual models (villages/buildings etc.), coding enemies or passive npc's, story/narrative/plot, any sort of systems and mechanics, rules, goals you know the small things...
