Should portfolio be in a pdf/document format or can we make it using real papers? For college portfolio round?

You can take either digital or a printed version of your portfolio, but it is better to take something tangible when you go for the second round.

It depends on NID. Sometimes they ask, sometimes they don't.

Yes you can add anything in your portfolio. Only condition being it has to be made by you. Medium is not a barrier.
