How to Complete a Child Support Program Financial Affidavit Administrative Support Proceeding Form

This video shows you how to complete the Florida Child Support Program form titled Financial Affidavit Administrative Support Proceeding. The Child Support Program sends this form to both parents to obtain financial information to determine child support payment amounts.
Some things you'll need to get started:
• Copy of your financial records for the past two years. For example: pay stubs, federal income tax returns, 1099 forms, benefit statements and/or other proof of income
• Health insurance information (if you have it)
• Information on any childcare expenses you pay
• Pen (Dark blue or black ink is preferred. Please don't use pencil.)
