the time when you have broke the game happend to me you can do it rarely if your going to a boss and there is an enemy in the same place. then you will fight the enemy and when you beat him you will kinda broke the game :)

the black screen boss glitch happend to me too, i can only guess it's when you trigger a random encounter with an enemy on the boss tile, cause that's what caused it for me.
