How do you make the assessment and plan in sentence form po? Can you please do an example for Suspected Ectopic Pregnancy, thank you!

Hello! You don't need to use sentences in assessment. Kung ano po yung babanggitin sa video, exactly as it is. I have included the sample complete SOAP note this particular practice video.
