While applying to a university should we contact the professor we are interested to work with or the student affairs office of that university? How important it is to have a published paper for the P.h.d application? Does MEXT accept that we take our TOEFL/IELTS exam a few months after applying for the scholarship? For example, whether it is acceptable if I say while applying in November that I booked my TOEFL exam on December, 1?

This depends If you are applying to the MEXT Embassy scholarship, then no need to contact right now, it's better to email after first screening result If you are applying to MEXT University recommended then it's vary university to university you can ask admission office of the university to know whether professor consent is required or not?

For Undergrad you need to apply through embassy track and which do not require to contact Professor first. You need to contact the University or Professor after getting selected in First Screening.
