My father in law had been on statins for 35 years, ldl supposedly under control. He had a quintiple bypass last year. It would have been interesting to see what his apoB would have been.

Suprising that the Canadian Guidelines, at least as seen here, do not also take into account the Trigylceride to HDL ratio, which has been shown in observational studies to predict Major Adverse Cardiovascular Events (MACE) and generally should fall below 2.5. To achieve that usually requires some combo of a significant reduction in dietary carbohydrates (sugars, flours, pasta, rice, white potato, etc), an anti-inflammatory diet in general, elimination of omega-6 rich seed oils, possibly certain supplements (nicotinic acid, Rx fish oils) not to mention regular exercise and substantial weight loss. Yea, that's a tall order isn't it. Maybe that's why it's left out?

Excellent review on the importance of measuring ApoB for assessing CVD risk. I see that you mentioned the importance of Lp(a) and that you'd cover it in the next video. However, it's been almost a year and i still don't see a video on Lp(a). Any plans to discuss this contributory risk factor and how to include it when assessing CVD risk?

Dave Feldman's research into this precise subject is being released during the first week of December 2023 - it is going to be interesting to see whether the advice given in this video is correct. NOTE: I am not saying that the advice is incorrect but am waiting for the Feldman point-of-view.

Fix both. Mediterranean ketogenic diet. High monos, unprocessed polys and low sats. It's important to know your apoB if on a high fat diet. You may find like me, you can eat eggs cheese, butter etc and still have a low apoB within the reference range.

Statins reduce ApoB, but do they reduce event risk?

This is okay where the diet is high in carbohydrates but for for someone in Ketosis? Probably not. Elevated HDL and LDL have been shown to extend longevity for those on low to no carb diet so in this case the ApoB would also be elevated however ApoB/ApoA1 would be very low indicating low risk. So understanding the patient as well as the science is important.

But, but, but, what about dietary interventions that drive Triglycerides through the floor and raise HDL and dramatically lower the ApoB/ApoA1 ratio a more inclusive maker than ApoB itself that of course is better targeted by drugs while no drug can lower Triglycerides or raise HDL effectively and why these excellent markers and their ratio are being surreptitiously sidelined as they present no financial incentives.

Its electronegative LDL and VLDL that is the real risk and these tests do not address that. Where is the research on electronegative LDL, there ARE papers but they are ignored.?
