I have seen all of those activities being adopted in big companies (in US and Canada), not small companies though. The problem with big companies and their extended inactive time, and fund, consuming bureaucracy make HR and managers lack behind too much (in various measures and aspects) while they themselves are administering the changes toward the main corporate goal.

Be careful with these team offsites as well -- there is a fine balance of an offsite being too "business-y" -- it really depends on your team composition, but some teams do better literally with a weekend away together with little business agenda than one where there is a lot of structure around defining the organization's purpose.
