Hey Neil, I am a resident at the beginning of my second year. I am still struggling with reading CT and your channel helps a lot!But I have also a question here: do you guys only use the templates or is this template just to get a better overview?

You mentioned that "people call the early arterial phase the pancreatic phase". Wouldn't the "pancreatic phase" be the delayed/late arterial phase instead? I'm not a radiologist. Just trying to learn the terms the radiologists use.

Interesting. As you mentioned, the pancreas protocol would be used to determine resectability of the tumor after initial diagnosis. But just out of curiosity, would a standard, IV contrast enhanced MDCT of the abdomen show an advanced mass in the pancreas? Thanks.

Yes. Most pancreas masses can be seen on a standard single phase CE CT. Mostly the pancreas protocol is needed to stage the tumor in in terms of it arterial and portal venous or SMV contact.

Some tumors like adenocarcinoma enhance less given the highly vascular nature of the pancreas itself. Others, like neuroendocrine tumors enhance more (hypervascular). Depends on the specific histology of the underlying tumor.

I also read that tumorfibroblasts act like a protective shield around the tumour tissue. Thats also the reason why Gemcitabin doesn't really work and will be given over 6 months.

​@@neslihangroll9232 Yes. Pancreatic adenocarcinomas are known for having a dense stromal component. The stroma is the supportive tissue around the tumor cells, and it can limit the blood supply to the tumor. In pancreatic adenocarcinomas, this dense stroma may result in reduced blood flow to the tumor, contributing to "hypovascularity." As a result, these tumors may show less enhancement on contrast-enhanced imaging studies like CT scans.
