Fever with retrosternal pain relieved by upright posture along with ST Elevation is pericarditis, which further classified into dry and Wet . Dry pericarditis is characterised by frictional rub with diffuse ST Elevation. White wet pericarditis has globular heart with muffled heart sound .

Great video. There is usually a presystolic sub during atrial contraction when you have friction rub, followed by a ventricular and then a diastolic rub- ie the rub itself has three componsnets.

Thank you for this helpful video. I noticed in the treatments section there was a specific contraindicated NSAID given as an example. Indomethacin is contraindicated since it may decrease coronary blood flow, I read this in my nursing textbook and just happened to remember reading since looking back on my notes.

If you learn mechanisms of action of drugs (pharmacology), then you will immediately know which medicament or group of medicaments you should or shouldn't use - and why. I mean it will make sense to you.

I think you are partially correct, and it would depend on the cause of the friction rub. If the patient was post-MI then this sounds right, but I imagine if the cause is related to acute illness or other inflammatory processes it would be okay. I don't have sources on this - just my clinical reasoning so correct me if I'm wrong. I think your point is a good one though and worth mentioning.

From what I've learned, NSAIDs are the treatment of choice in pericarditis EXCEPT in *post-myocardial infarction* pericarditis where there are special considerations: --> *Early (1-3 days) post-MI pericarditis (peri-infarction pericarditis):* give *supportive care* only. _(Avoid NSAIDs which can impair collagen deposition & increase risk of post-MI complications e.g. LV aneurysm)_ --> *Late (2-10 weeks) post-MI pericarditis (postmyocardial infarction syndrome* a.k.a. *Dressler syndrome):* give *aspirin* only. _(Other NSAIDs associated with increased risk of repeat MI and bleeding complications in patients with CAD.)_ --> *Pericarditis due to all other causes* (idiopathic, viral, etc.): *NSAIDs* are treatment of choice _(Other options: colchicine, steroids)_

After my first dose of Pfizer last Monday I've been having chest pain (stabbing and burning on and off and all over). I'm only 17 and so scared. I've been to the ER twice and they did an X-ray the first time, Bloods and ECG. The 2nd time they did ECG and bloods. They said I didn't have myocarditis but what about pericarditis? They sent me home despite a low blood pressure and my symptoms. I went to the GP twice and they told me it was just anxiety... but its not. My wrists are also hurting and knees, my left hand (the arm I had it in) burns... I just want answers. I have my own stethoscope so was trying to listen to see but I can't tell.

Oh I've also noticed my fingers and toes are cold and my hands are blotchy. I believe this is a circulation issue. I've also had a rash on my chest

I’m sorry your going through this. Insist you have a D-dimer blood test, it shows any thickening of the blood.

@@katBerk I was given no answers. I still get chest pain. Its been getting colder and I notice its worse in the cold. I had another ECG as I got covid and the pain became worse and my untaxed sister who got covid too looked like she had pericarditis. That ECG showed a sinus arrhythmia. I'm supposed to do a 24hr holter test and another Echocardiogram but I can't afford it. I have gotten another X-ray after having covid too, I don't know the results of that yet. Covid itself didn't affect me badly at all.

Get an echocardiogram done as well as a CT of the heart scan. As well as insisting on the d-dimer blood test (you have to specifically ask for it) I previously mentioned. Please don’t have any more jabs.
