Bent up bars are provided in a beam in order to resist the Negative Bending Moment generated at supports. Following is the BMD of a continuous Beam: At the portion of Negative Bending Moment tension is on top portion of the Slab so Reinforcement is required in top portion of the Slab.

No it is just for rectangular beam. It does not include slab thickness.
