if the patient was vomiting we can use nasal mask? but if the patient was unconscious we must do intubation?

If the patient is vomiting the risk of aspiration is very high while using NIV. You would wait until vomiting had finished before using any positive pressure non invasively.

You could add supplemental oxygen if required via an additional valve connected to a tube at the patient end of the circuit shown here. Otherwise NIV uses room air only and positive pressure rather than 02 to improve ventilation of the patient.

A couple of things: the 'elephant" tubing shown first is not "oxygen" tubing but for room air. Supplemtal oxygen can be added via a narrower tube and valve. Secondly there is no mention of which mode you are running on the 3+.I would assume this would be the first thing to decide on before adjusting settings. Modes on the 3+ are PCV, PSV, CPAP and IPPV. You are presumably in PSV mode in this video as you are not running an IPPV circuit and are adjusting EPAP.

The mode would be determined by the type and cause of respiratory failure and is a critical decision.
