thats usually when the connection from the player is disturbed. restart their Minecraft client usually fixes that problem

In my knowledge, not sure but if you don't have that much in your server, you can do a manual backup then transfer them to a new one via sftp

If your minecraft account is cracked, then you have to go to server.properties and under online-mode=true. Set that to false and save the properties.
