What if my toddler just wants to grab the paper when trying to show the visual?

That's ok! They may not be ready for that step yet. You can always just say yes/no along with nodding and shaking your head.

I found this when trying to get some things to show some parents. Very nice, but what do you do if child has no pointing skills, no nod, no reaction to yes and no from others?

Then that child probably isn't ready to work on yes/no questions. I would probably start with establishing joint attention and being able to communicate basic wants and needs like requesting something they want or refusing something.
